Some time ago, Dr. Dimitrakov mentioned a German product being used for neuropathy; what was that? Also, at the suggestion of my pain M.D. I've started to use alpha lipoic acid and taurine for nerve pain; thoughts? JP :?:

The name of the product is "Milgamma N". Ingedients: lipid-soluble vitamin B1 (benfothiamine), vitamin B6 and vitamin B12.

Dr. Dimitrakov, do you have any thoughts/experience as to the efficacy of Milgamma N or other supplements, including alpha lipoic acid and taurine, with respect to CPPS? Thank you. JP

Any sense of how many of your patients have taken it, how many it helped and what improvement in the NIH-CPSI was seen in those patients it helped? Did they take it as first line therapy or only after other treatments (Elavil, Neurontin) failed?
Milgamma N has been quite well-tolerated by all patients that have taken it so far and has been effective for neuropathic pain control. I have no experience with alpha-lipoic acid or taurine but theoretically they seem to make sense.

So far, about 50 patients
how many it helped
About 70 %
and what improvement in the NIH-CPSI was seen in those patients it helped?
Mainly in th pain-related domain
Did they take it as first line therapy or only after other treatments (Elavil, Neurontin) failed?
They took it after other treatments had failed.

It will be undergoing evaluation in a double-blind placebo-controlled study soon. The problem is the company is not interested so it will be a bit difficult to fund the study.

Is there something special about this supplement that makes it better than plain old B vitamins? Could a patient get the same effect from simply taking the subject B vitamins? Thanks. JP

====It is lipid-soluble vitamin B unlike the plain old vitamin B combo which is water-soluble. Since nerves are rich in lipids and lipids are soluble in lipids, the lipid formulation has greater efficacy than the water-soluble one.

Dr. D in your opinion what supplement's would be similar to Milgamma-N? You also mentioned awhile back something about Tribulis for dialated seminal vesicles could you elaborate on that a little more please. I have been diagnosed with dialated seminal vesicles + chronic prostatitis / chronic pelvic pain syndrome and I also remember you mentioning that that this could cause back pain aroung the kidney area. This has been a problem now for 14 months and I am desperatley looking for something to help especially with the back problem. Everyday same thing never gets any better no matter what I try Yoga, Stretching, Whirlpool, nothing.

Well, it's not the simple combo of the vitamins inside (vit B1+B6+B12) but rather the fact that you get the lipid-soluble formulation (as compared to OTC formulations which are water-soluble). Since lipids dissolve well in lipids, this formulation has been shown to penetrate nerves better than the water-based one (sincenerves are rich in lipids)

Tribulus has properties similar to natural testosterone and since smooth muscle contractions in the prostate and seminal vesicles have been shown to be under testosterone control, it probably stimulates contraction in this way.
