In fact, I'd be interested to read what justification your uro has in the literature for suggesting a prostate massage under anesthetic. As far as I know, there is none.

That study seems to reflect the quick and regular 'bend over and finger up the bum' massage with antibiotics which I don't think is the same thing as my Uro is proposing. Still, I will question to him as to exactly what he intends to do.
Let's beg to differ for now, shall we? Personally I don't see it doing much good but, hey, you never know? In the meantime I'm watching my diet, taking any accredited quercetin product and will be embarking on stretches and relaxation shortly.
I will, of course, run a mile if he starts going on about injections.

No problem, do as you see fit, but I urge you to find out on what empirical basis he suggested this procedure. Or did you suggest it to him? Some doctors are very susceptible to patient pressure....

No, not really. 30% is the usual placebo response to treatment. IOW, about 30% of people will get better over the course of a study and its (usually short) follow-up period, even with a placebo pill. In addition, you have the pressure applied to the levator ani insertions during a prostate massage that confounds the results.
Certainly not worth the risk of a general anaesthetic.

I had my latest appointment with my Uro on Friday. We talked about my symptoms and how I was doing - I told him I was very slowly improving from month to month (which, the odd setback aside, I am) and described my remaining symptoms. He confirmed that he thought I had pelvic pain syndrome and felt that as it seemed to be gradually improving we should leave things for another four months and see where we were then as his experience is that - although there are no guarantees and some people don't improve - it generally fizzles out over time, perhaps not completely but to a point where most people simply aren't really bothered any more by their symptoms.
I asked about treatment and he mentioned that antibiotics are used first because a lot of men respond to them, similarly anti-inflammatories are then used for the same reason. He also mentioned that we could look at prostate massage and hydrodistension as these sometimes helped people although, again, there were no guarantees. I asked about benzos and Elavil and he mentioned that he tried to avoid prescribing benzos because of their addictive qualities. However, if things hadn't improved then this was an option. As for Elavil he said we could try it if I wanted but in all honesty it would probably be better to see how things progress naturally first. My urinary flow and retention are excellent though - my flow rate is "as good as it gets" for a man and my retention is "nothing" so that's a relief.
I then asked about kegels as my main symptom now is the annoying feeling of having what feels like a drop of fluid trapped at my meatus. He said the kegels are useful for incontinence and sometimes for OAB but not really useful for this condition.
Finally I asked about muscle issues and although he felt my current issues are more likely a result of inflammation - either pre or post surgical - that stretching and exercise certainly wouldn't harm me.
So I return in four months to see where I am.
Current symptoms are:
a) Mainly pretty symptom free (say 75%-85%) up until about lunchtime when I have my second pee of the day as well as a dump whereupon...
b) Feeling of having a drop of fluid stuck somewhere in my glans. Can last until next urination or sometimes just decreases to feeling like the urethra is swollen. Prob 50% better than pre-op.
c) Sometimes a feeling that I have to poo when I don't.
d) The occasional bit of ED
e) Feeling tired quite a lot
f) Sometimes a burning feeling in the prostate are later on. This replaces the 'drop' feeling. It seems to be one or the other. Again about 50% better than pre-op.
No real pain to speak of.
So, I seem to be getting there slowly. My protocol - if you can call it that - is 3 any accredited quercetin product a day, 1 Glucosamine and Chondroitin tablet, Vit B and a St John's Wort pill for mild anxiety and ejaculating about 3 times a week.

DW advises against Kegels. Your growing symptoms and fatigue as the day progresses is typical, and will slowly improve over time. If you can siesta, do it. Keep up with the stretches and moment-to-moment relaxation, and the Quercetin. Glad it's moving in the right direction for you!

Cheers Sleeper I think the Elavil when your ready will help as well. As far as Urine, I try to gently use my PC muscle to release the remaining urine and gently press my perineum to get the rest out so I dont flex those muscles to much.
Time to hop on those electric drums and let loose in a relaxed way( or should I say gently sit and let loose).

Ya I like the TD-6 as well. Every time I hit the music store, I always stop at the electric drums and have at it. I know how you feel, temptation all the way. One of these days I'm going to dive into it and do it. If only my studio stuff I have been doing for groove addicts takes off and I get some cash for it. Working on doing a recording and trying to get into film score and all that crazy music stuff with my Brother. Keep your fingers crossed

Thought I'd better update this as it's about six months on.
Things continue to gradually improve - I would say that I'm now about 70-80% most of the time and about 90% in the mornings most days. I do, however, get the odd bad day or two where I'm about 50% but compared to pre-op days it's a world of difference. There are also days - particularly if I'm away on holiday - where I am at 95% all of the time.
My main symptom remains urethral irritation. I no longer have any ED issues and the feeling of sometimes having a full bowel has faded a lot. I also seem to have more energy and am now working on diet and fitness. The irritation ranges from hardly noticeable to quite annoying with the former state being the more prevalent.
I continue to use MSM, Quercetin, Glucosamine and Chondroiton and Omega 3 Oil, relaxation and gentle exercise all of which seem to work for me. I've discussed the situation with my Uro and his opinion is that we should continue down this path for now as my symptoms do seem to be burning out with time and conservative treatment.
I guess the bottom line is that, as has been stated, it's a slow process but we tend to get there in the end.
