MY PT UPDATE (Rufus Case)
It's been a while since I posted and thought I should submit an update.
Do I have good news?
Well, hard to say...
I am feeling better but very much afraid of returning to "normal" activities for fear of the pain returning at a higher strength but ejaculation is no longer making me bedridden and considering disability as a means to go through life.
But despite seeing a PT (who in retrospect didn't know wtf she was doing) and later getting therapy from a chiropractor (who knew exactly wtf he was doing) I still feel there is a component of therapy which I have not addressed and that my friends is the internal work.
I'm considering getting a wand and trying it on my own since it's so difficult to find anyone with a successful track record of treating this condition. Going into this blindly is a mistake, I know, but I feel that I have little choice.
The quack PT I saw and spent a considerable amount of money, I feel, really didn't help me much. The chiro, I feel, has stretched me out and addressed my TP in the psoas and other lower abdominals that have made me feel relieved 90% of the time.
And after making what I'd think is progress I get a set back here and there which although is a part of this condition, can sometimes rob me of the notion that I'll ever be free of it....
And to make matters more interesting, ever now and again my wife and I have oral sex and it seems after she performs on me the tip of my penis burns. It has subsided but the mental toll it took on my was bad...
Here I was, thinking on how great things have been, my symptoms manageable, I was doing stretches at home and slowly doing things I used to enjoy and NOT thinking about the problem itself during all waking hours and then the tip thing which only seems to surface after engaging in oral sex...
I'm trying to find some correlation between chronic prostatitis / chronic pelvic pain syndrome and this oral sex/tip o penis pain and the SP. Is there any? Is this oral sex thing in my head?
Have I truly made progress or would I be in the same position with the pain at a 1-2 level had I not made any attempt to stretch or address TPs?
These are questions I ask now and again but it always goes back to the original question which is how did I get this in the first place and if this is as common as I think it is, why isn't every doctor schooled on how to cure it?
I'm just confused....
Hope you guys are doing well.
Rufus

Age:37 | Onset Age: 35 | Symptoms: pain in testicles that comes and goes that also switches sides-trying to find a pattern as to "why" this happens/Rectal Burn at the 6 O'clock position at my anus which tends to flare around the periphery of my anus typically after defication but no perineum pain-no "golf ball" pain. Pain in testicles worsens as I sit but not always...Pain is not always present but depresses me. Pain used to be at a very high level but has subsided in time to a low level but has not gone away. Pain used to flare after sex but not as much as before although it still like playing russian roulette./
/Lack of SleepHelped By: I haven't found anything that has helped to my knowledge. Tried many many sessions of PT, accupuncture and chiropractic care but still have not found steady relief that takes me back to a pre CPPS state.

Re: MY PT UPDATE
It can take a very long time to get better. I got slowly better for years, with many setbacks. But the trend was inexorably upwards.Rufus wrote:And after making what I'd think is progress I get a set back here and there which although is a part of this condition, can sometimes rob me of the notion that I'll ever be free of it....
This is often reported, and has even led to relationship troubles, with men mistakenly blaming their partners for "infecting" them. What I believe is happening is that men often tense up their pelvic muscles to an amazing degree during oral sex, to make the penis more sensitive to the rather mild sensation of oral stimulation, as compared to vaginal stimulation. This gooses the pelvic nerves and sets off another round of pelvic myoneuropathy. And to make matters more interesting, ever now and again my wife and I have oral sex and it seems after she performs on me the tip of my penis burns.

Thanks for the response which clearly makes sense but wasn't the root cause of your chronic prostatitis / chronic pelvic pain syndrome related to diet?

I was a diet and stress case, with both contributing mightily. I'm not referring to my own case above.

Hey G...
Remember to gage your recovery by months and not days. I can remember your early posts of sheer terror and pain. 90 % better at times is a great sign that you are on the right path.
I'm still trying to figure out in terms of ejaculation what works for me. It seems 2 to 3 at most is about right for me with no discomfort. I have noticed If I have sex 1 to 2 times per week with my Wife, it feels the best for both of us. No over doing it on both sides here seems to be the way to go for me and for my wife.
Now if I can just stay away from those free big tit websites when she's out and about and my kids are sleeping, I think I will be better off.

I encourage anxiety prone UCPPS people to consider L-Theanine
Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times, poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.

Switching Sides
Is it the nature of trigger points to cause pain in the scrotum to switch from side to side?

Sports Hernia
Has anyone here ever been evaluated for a sports hernia?
It's not a hernia in the traditional sense at all but rather tears in the lower abs that can cause testicular pain.

That's something I've never heard of before, and since it's very difficult to diagnose (diagnosis by exclusion), it would have to be a consideration for anyone with testicular pain.
I guess you'd be able to rule it out if you have post-ejaculatory pain (hallmark of CPPS), painful urination, or any of the other symptoms specific to chronic pelvic pain.

This is a reponse I received from one of the three doctors that I can find that deal with sports hernias.
_____________________________________________________________
To answer your questions:
a sports hernia can cause pain after having sex.
a sports hernia can cause pain in sitting position and can become worse after a long time of sitting
a sports hernia shows a typical pain radiation to the adductor area and often as well to the testicles.
It is typical that a sports hernia can not be seen with CT or MRI, because you are in a recumbent position .
The best way to find out, if it is a sportsman groin ( sports hernia) or not, is the digital examination in standing position and the ultrasound.
_____________________________________________________________
The two general surgeons I saw way back when didn't know what a sports hernia was and neither did a doctor who I recently saw that runs a clinic for chronic pelvic pain.
So far I"ve only been able to find three doctors who specialize in Athletic Pubalgia a.k.a. sports hernia. Why does this appear to be something that most doctors are not familiar?

But not the burning, centrally located pain that radiates to the tip of the penis. I have never seen sports hernia cited in the differential diagnosis of this pain.Rufus wrote:a sports hernia can cause pain after having sex.

Sports hernia is also known as Gilmore's Groin. This should help you differentiate it from CPPS
http://www.physioroom.com/injuries/groi ... a_full.phpphysioroom.com wrote:The symptoms of Gilmore's Groin are characterised by pain during sports movements, particularly twisting and turning. This pain usually radiates to the Adductor muscle region and even the Testicles, although it is often difficult for the patient to pin-point.
Following sporting activity the person with Gilmore's Groin will be stiff and sore. The day after sports activity, getting out of bed or a car will be difficult. Any exertion that increases intra-abdominal pressure, such as coughing, sneezing or sporting activity can cause pain. In the early stages, the person may be able to continue playing their sport, but the problem usually gets progressively worse.
Pain in the groin and pelvis can be referred from a number of problems, including injuries to the lumbar spine, the hip joint, the sacro-iliac joint, the abdomen and the genito-urinary system, so diagnosis of Gilmore's Groin requires skilful differentiation.
The diagnosis of Gilmore's Groin is based on the patient's history and clinical signs. The most notable clinical sign is widening of the superficial Inguinal ring on the affected side, which can be palpated during the physical examination when the scrotum is inverted with the doctor's finger. Typically, there is specific pain on coughing and sneezing, as well as sitting up and squeezing the legs together.
