Fellas,

Webslave said in another post:

"Rubbing on the prostate usually also means you are massaging the levator ani insertions, so there is a confounding factor at work if any improvement is seen."

Would those insertions be on the perineum floor area or to the sides around the prostate or above?

I still have tender areas on the perineum floor and above the prostate to the right and left.

I've tried to study the damn pelvic floor, but I always get turned upside down and have problems locating certain things. Sort of sad to admit.

Thanks,

Ss

Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times, poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.

It's not the muscles of the pelvic floor, it's the nerves that innervate those muscles and the genito-urinary tract, as well as the trigger points those nerves create in the muscles. Read the "What is pelvic myoneuropathy" link in my sig below again.

Thank you Mark, that's why I'm so grateful for this site, because it's good to review everything and go back to it as needed to remind myself of what's happening here. Much respect!!!

Burning in my urethra / perinea is my only real symptom yet its darn painful at times. Is this more to do with nerves then or trigger points or is it inflammation?

Scooby / Webslave do you know or is there not an exact answer?
Age: 29 | Onset Age: 26 | Symptoms: initially severe burning throughout pelvis - much better; muscles spasms all over region, back pain, chronic IBS (much better) | Helped By: Psychotherapy, SP stretches, swimming, accepting the condition (understanding there is treatment); stress/anxiety management; possibly basic quercetin; valerian root, vit B & calc & magn supplements; gluten avoidance & especially dealing with chronic constipation. | Worsened By: stress / anxiety, too much sex; repressed emotions; personality goodist traits. 80% better

I dont think we have a definitive answer to that, but its my belief its caused by mast cells and nerves, caused from the tight muscles/tps. I have the burning too, but only when the pelvic floor won't drop (meaning the tps are aggravated and flared meaning the muscles are tight). I also think if the floor won't drop correctly during pissing you are pushing urine through a smaller opening than it should be thus resulting in more inflammation. This is just my though process on it.

I have noticed this too. When I relax and pee it comes out good, when I'm tight it sprays all over the place and splits and then I feel worse after. In the begining I tried pushing harder when it came out slow because it felt like something was blocking it, of course doing this I was making thigs worse.

Best thing is think about something else close your eyes and smile why peeing, sounds stupid but worked for me.

This is my main complaint followed by frequency.

Greg,

I think it's all of the above. Re-read Mark's signature on pelvic myoneuropathy like I did to review what's going on here. Stress/ anxiety/ tight muscles/ injury and TP's can cause neurogenic inflammation and release substance p and other neuropeptides which like Scooby said, cause mast cells to degranulate and release histamine, serotonin and prostaglandins. This can make the whole g- tract more permeable. So you get all of these symptoms like we have.
