Hi all, I've been doing PT now for about 8 sessions and for the moment I think I have noticed a small improvement, although I still seem to be stuck in the same cycle of one good week followed by one bad week.
During my PT my therapist has found that in general I have a a lot of muscles / tendons which are damaged pretty much throughout the body and it looks like this has been caused by stress. She also found that my knee is damaged and this has had repercussions on the entire left side down to the foot and up to the pelvis. This damage has passed to the right side too in the pelvis region and the abdomen.
2 Days ago she tried to find internal trigger points, although I didn't notice anything particularly painful she did find a point she said felt like it was damaged.
She also found a point on the underside of the penis which transmitted the exact sensation I feel that I need to wee and pain on the tip of the penis. Since that day it has felt sensitive and I guess because it has been moved about so much. She was surprised the Urologist has not check the urethra although thinks it is again fibre damage and probably something the Urologist will not be able to cure. Have to say it was quite odd having a women other than my wife playing around down there but I think she has found something important in my case. Does anyone else have this type of TP (I have seen it mentioned in D. Wise book)?

What the heck is "fibre damage?" The frenulum of my penis is the center of all my discomfort there as well...

damage is a strong word, I like to think of it as tight muscles, nothing more, nothing less.brettboz wrote:Hi all, I've been doing PT now for about 8 sessions and for the moment I think I have noticed a small improvement, although I still seem to be stuck in the same cycle of one good week followed by one bad week.

As for the area under the penis that recreated your symptoms, that I would think would be a VERY important area for you to try to work on. Stretch, heat, trigger point release.

YEs damage is probably wrong word, what I mean is it's not like its supposed to be. Also when I say fibres I kind if literally translated it from what my PT told me (in Spanish), she means tendons and muscles. In yesterdays session she found a further point which transmits pain to the same area, it seems that with time we are gradually finding the worst points to work on more.

My wife often comes to my PT sessions. She and the PT have become good friends, and the PT has excellent advice about some of my other medical problems.brettboz wrote: Have to say it was quite odd having a women other than my wife playing around down there
Having a woman do internal work while chatting about her children to my wife is a little surreal.
I am not a doctor too!
Age 64, sudden onset 5/06 during bout of flu, colitis, lung and liver problems. Pain in penis, then moved to rectum. No urinary symptoms. Use Lyrica, Elavil, paxil, Xanax, morphine, vicodin, relaxation, pads for sitting, 100+ PT sessions. Sensitive to gluten, milk protein, soy. I have CFS, anxiety, GERD, Barrett's, colitis, kidney stones, one of which referred pain to penis.

The internal work she has done on me has come up with very little in the way of TP's. Seems that most of my problem is exterior. My cycle still seems to be one good week one bad. The bad week is just starting and I can notice the first signs of it getting worse, this time I'm going to try to stop it with Valium instead of NSAID. The latter is starting to have bad effect on my stomach.

Not saying you have internal trigger points, but the first two PTs that saw me said I didn't have trigger points. One visit with Tim Sawyer revealed a much different state. And I know he found them because I was in extreme pain and discomfort with little touch.brettboz wrote:The internal work she has done on me has come up with very little in the way of TP's. If your symptoms dont start to improve with just external work id seek another opinion with someone very skilled in this stuff.

I encourage anxiety prone UCPPS people to consider L-Theanine
Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times, poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.

Trigger Points
I started doing self-administered external trigger point release along the opsoas about 3 weeks ago and got immediate relief every time I did it. Now I am thinking the effectiveness may be dimishing over time. Does anyone know if it's possible to do too much trigger point release on oneself?.

Re: Trigger Points
I believe you can. I think when they are really tight you can only do it about every 3-4 days, you have to give it time to get the soarness out. Think about it, have you ever had soar hamstrings? You try to bend over and they are tight as shit. Well same thing here, if you overwork them, ie trigger point release, and I believe when you do this on chronically tight muscles it is a form of exertion, you need to give time to relax. So do a session then wait 3-4 days, in that time stretch and stretch some more.bshaw99 wrote:I started doing self-administered external trigger point release along the opsoas about 3 weeks ago and got immediate relief every time I did it. As the trigger points dissipate, I beleive you can increase the frequency to keep things extra lose. This is where I seem to be right now.
I'm hoping that through more frequent sessions now that I'm at this point I can force the muscles into the normal relaxed state more often.
But, at first I think you can certainly over do this, and you probably have.

Did anyone attempt internal trigger point release without being shown how to beforehand. The headache in the pelvis book shows where to go but I wonder whether its something that needs to be shown...

If you have a qualified PT expert in pelvic floor dysfunction, ask them how to show you self internal work.brettboz wrote:Did anyone attempt internal trigger point release without being shown how to beforehand. For my self when I do it, I sit on my toilet and gently use my gloved middle finger and relax the sphincter as I go inside. I go from the front, between my legs. I gently press on the muscles around the prostate and below it. I do this for only a few minutes. This mimics my LUTS condition and discomfort in my perineum and of having to pass urine with out touching my prostate. This tells me that this is a muscle issue thats causing my problems. I have backed off and do it once a month right now while I focus on relaxation and stretching and supplements along with Aleve (naproxen) as needed and I still take my uroxatral and 10 mg of Elavil which all seem to help me as I stand at 90% better from my worst of times at about 3 or 4 months into my onset in Feb. 2006. I'm not a doctor....
