BNI Bladder Neck Incision

Had a meeting with my consultant today. I had misunderstood about the pressure in my bladder which is not overly high but is in fact in the low normal range. This means I fall into the equivocal range for obstruction(Schaeffer II). However, the video urodynamics showed that there is clearly a problem with the bladder neck and my urine flow is very weak (11 ml/s on first void, 7 ml/s with catheter) thus a BNI is definitely the way to go. He feels this is more likely to succeed than not so, whilst he could not guarantee it, he thought it should resolve things.

We've set a date of 26 April for the operation as this is the first formal slot he has. However, he should be able to get me one earlier than this and will e-mail with details once this has been sorted out. Kind of hoping we can get one in March.

He said that I should expect retro rather than not - it will be a deep unilateral incision - , however the chances of impotence and incontinence are practically nil. The only other risks are the possibility of some bleeding and infection.

Pretty good. PT took any pain I had away (apart from my bladder grumbling because it's not quite emptying) and helped with some old rugby injuries too. Unfortunately it can't do anything for the narrowed bladder neck and it's going to have to be cut.

Generally optimistic as the procedure has a high success rate. I'm hoping to be back on my feet by the end of March.

My surgery took place yesterday. It was an overnight stay with a catheter in but no complications so I was released this morning.

Urine flow has increased quite dramatically but other symptoms will take longer to resolve (two to eight weeks) as the wound heals and the bladder settles down. My surgeon felt the operation went well and commented that my bladder neck was "pretty overgrown" - he seemed happier after the surgery than before it.

I have an appointment with him in six weeks where we'll review progress and see where we are with things.

Great to hea Sleeper. Impotence is very rare. I asked my Uro of the possibilty and he said that of all the operations he had done, which are countless, not one with impotence.

How long do you need to leave or had the catheter in?
Age 43: Onset June 2005, Initially: perineal pain, discomfort in penis, very sensitive, sometimes burning. Red around the urethra on penis. Increased urination during night, problems with stream, sometimes constant urge to urinate. Exercise and moving around and laying down helps. Sitting down and squatting makes it worse. Improved to about 85-95%. Symptoms usually mild pain near belly button and slight urge to urinate.

Catheter was removed the next day. The bleeding was minimal so there was no problem with it. Having it in is a bit uncomfortable but not painful - I actually had more discomfort from the cannula they put in my hand for the anaesthetic as it bruised a bit!

My symptoms are a bit better and my Doctor thinks they will continue to improve - she says it's only been ten days so there will still be swelling and scabbing in the urethra and bladder neck. She's taken a urine sample and sent it off to check for any infection but doesn't think there will be.

I will almost certainly have retro, however that's not a huge issue to us as we're looking at IVF anyway and they can extract semen from urine very easily.

My Urologist said that the catheter had to stay in for at least 2 weeks to prevent the bladder neck from closing up again while it heals.

There should be a 15% chance of retro.

Your Urologist sounds a bit odd! Catheters aren't left in any longer than necessary to reduce the risk of infection. Usually they are removed within 48 hours unless there is unusually heavy bleeding or complete retention. It could be that he meant that it would be kept in if you couldn't pee after the operation. This is usually the only circumstance in which it's kept in for a longer period.

The chance of retro depends on the type of operation. A shallow unilateral cut has about a 1 in 5 chance of causing it, a deeper longer cut or multiple cuts has a much higher chance. My Urologist said that if you don't get retro in this kind of operation it usually means the procedure has failed.

Like I said, it's not a huge problem for me anyway - my wife is 44 and it looks like IVF is our best option anyway.

Update after 3 weeks:

Urine stream is still very good - easily over 20ml's peak. My other symptoms are starting to fade a bit. For the first time in over a year I can actually differentiate between the uncomfortable sensation in my urethra and needing to pee. The sensation itself is slowly improving too and is considerably less intense than it was pre op and there are times when it's not there at all now.

I saw my GP and she said that the urine stream is a good sign as if this improves generally everything else does. It will, however, be a while before I see all the benefits as the wound takes at least six weeks to heal and the bladder needs to relearn its functions again. In the meantime I've to provide a urine sample in three weeks to check that bleeding has stopped (I still have a high RBC count as you would expect after the op).

It's still early but I'm fairly optimistic that this will sort things out.
