Pain on or after ejaculation: do you experience this or not?

Post by Gav »

Something has been bugging me for a while. The amount of people that talk about ejaculatory pain with prostatitis and in the broad definition (on this site) of prostatitis one meeting came to an observation:-

"Pain after ejaculation is a very specific complaint that distinguishes prostatitis from BPH or normal men (Litwin and others 1999)"

I have never had any ejaculatory or post ejaculatory pain, not even any sensitivity since problems started in the last 4 months, even when I was at my worst a few weeks ago and was really struggling. And wondered if I am in the wrong place here.

Anyone else there not suffer with ejaculation pains?

I know the above quote doesn't necessarily represent the views of folk on here but it was an interesting quote from that page given my symptoms so I wanted to bring it up and ask you more experienced guys. Thanks in advance for any help!

That quote from the last century if you will says a lot. I'm sure there is more to that article, still it's a quote from the dark ages even though it's only 8 years old, but from what we have learned through pelvic myoneuropathy, (please re-read that broad definition again) and neurogenic inflammation, that 1999 quote is "old school". Pain is a key component of this condition. However it is not the only symptom. Many men unfortunately have pain with sex and some have mild discomfort upon ejaculation that can wax an wane. Some men have no pain with sex. Some men have no pain at all and complain more about LUTS and discomfort in the perineum etc. CP/CPPS is a very customized problem with so many nerves that when sensitized, can cause all sorts of symptoms. Hope that helps. :-D

Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times, poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane. Makes worse: sitting for long periods, stress, over focusing on it. Currently 95%-98% recovered. Stay positive, relaxed and control your anxiety.

I interpret this statement as follows:

Pain on ejaculation rules out BPH. However, this statement does not mean that all prostatitis REQUIRES pain on ejaculation.
Age: 32 | Symptoms: Twingy pain/muscle spasms in groin area (gone), tingling sensation at tip of penis (has resolved as of now), urinary frequency (resolved as of now), thicker/decreased amount of semen (resolved as of now). Makes better: Stretching, Relaxing pelvic muscles, moment to moment relaxation, not sitting with legs crossed, Elavil, quercetin. Makes worse: Sitting, stress, more than one orgasm/day, constipation, manual labour

I have purchased HITP and am practising the stretches every other day. I am also practising forms of relaxation throughout the day and trying to relax as much as I can in terms of the muscles in my pelvic ares.

I am currently using a prostate massager every 3 days to simulate a Digital Rectal Examination (not a full massage) as I noticed with each DRE I was having my symptoms vanished for few days. Over the time I have down this (approx 3 weeks) my symptoms have improved (I can actually go on average about 4 hours between peeing) and I have actually started to get some drainage from the DRE (50% of the time). I do not know if I am getting some kind of myofascial release or if because my case is still quite new (diagnosed after symptoms started abruptly roughly 3 months ago after I had a ureterscopy to remove a kidney stone) there may still be a small small chance I am dealing with something bacterial, unlike the majority of this board.

I am also using Ciprofloxacin still along with this but will be discontinuing with its use in 3-4 weeks after I run out. This is Cipro's last chance with me to be honest. I am also using an alpha blocker (Tamsulosin) and all of this seems to be working better with time. Once the Cipro is done and if things start to stop getting better, I know there is a PT in Scotland and that will probably be my next stop.

I am also gluten free at the moment to see if this helps. It seems to have calmed down any anal pains I have and I no long er have any constipation.

Overall, things are much better than a month or two ago. Whether I do have chronic prostatitis / chronic pelvic pain syndrome or a slight chance of something bacterial I guess I will find out soon enough.

Sounds like you are doing just about everything you can do.

The prostate massager is something I'm not sure of, but if it's working for you? The only reason I say this is because if the prostate is already irritated, I'm not sure this is something I would mess with. The muscles around the prostate can be the muscles that can cause your frequency and discomfort. You may be unknowingly hurting your prostate to be working it 3 times a week like that. If there is harmful bacteria, you don't want that stuff in your blood to cause some problems. Again, what do I know? I have not heard to many people talk about massaging their prostates with a massager? At least you are doing the Cipro for 4 weeks to rule out infection. This is a common approach. I'm not a doctor :)

This is actually my second go with Ciprofloxacin after I initially used it with great success (no symptoms at all) for two weeks, but then my specialist asked me to lower the dose and symptoms returned. I then tried doxycycline which worked in patches but did not much else. The combines use of the massager and Cipro for longer this time along with stretches, relaxation and a lot of patience is where I am going currently.

Thanks,

Gav

I asked about the microbiological tests of your EPS and/or urine.

Just my opinion and I'm in no way qualified to say this because I'm not a doctor....

Time to stop the antibiotics, you will do more harm then good if you have gone past 4 to 5 weeks. If you are past that time, you are not doing your self any good. For reasons that are still poorly understood, the AB can have an anti-inflammatory effect. That's why you have had some relief. Get your free sample of any accredited quercetin product and keep doing everything else you are doing. Still real unsure about this prostate massager you are doing? I think you are dealing with chronically tightned muscles and possible food intolerances here. You never know til you try it all, right? Take care...Peace and out, it's the weekend!!!!

That is good advice. I will let this course of antibiotics run its course and then it is any accredited quercetin product and some real thoughts about seeing the PT in Scotland. I shall just say again I am not using the Prostate Massager for a vigorous prostate massage. I know the dangers that lie within that, I am simply pushing it in fully and then removing it, like a DRE. I am not prodding or pushing against the prostate with any force like a proper massage.

I have also purchased the Trigger Point Therapy Workbook by Clair Davies and it is on its way as part of my anticipation of what lies ahead.

Enjoy the weekend folks.

I have to add in, STOP the antibiotics asap and do not try to relieve the pain with an anti-inflammatory they are also bad stuff. Try to follow the example of most of the others or you will end up paying for follow what most of the uro's tell you to try. I have had chronic prostatitis for 4 years and wrecked my body by trying to stop the pain with meds. Please listen to Webslave and the other with success stories they will stear you in the right direction.
