Need Help

Post by Link »

Been living with urinary frequency, and random pain (burning, pressure) in perineum area, and penis. This has been going on for 3 years off and on. I would say I'm in the severe category when on.....

Some odd things I noticed I need help on....
1)Whenever I'm in pain if I get an erection it's an automatic off switch I feel fine to return to pain when non erection??

2)When ever I get sick, and if in "flare" pain goes

I was wondering if anyone else noticed this????????????

Background:
I have had:
Urodynamic Test
Multiple Cysto
Cysto with Hydrodistention
Sonogram
CT SCAN
X Ray
Diet Change

I have used the following meds:
Cardura
Hydroxizine
Baclifin
Elmiron
NSAID

Things that do work no sh##!
1) If feel bad put a bag of Ice under testes and on perineum for 30-45 minutes this takes the swelling down and pain away immediately
2) Immediately Take an anti-histamine preferably 50Mg of Hydroxizine. NyQuil is a charmer for me I don't know why but overnight it sets things straight.
3)Fast for 1 day, then introduce mild items
4)No carbonated drinks, alcohol, caffeine, spicy food, bad oils (stick with cold pressed olive or canola), fried foods,or high glycemic foods (Potato, Sugar, Bread etc..)
5)Take supplements Quercetin, Flax Seed Oil
6) If things get to bad I take 800mg Ibuprofin
7) Exercise .5 hour a day (reduces stress increase blood flow)
8) Stretch this is key 2x a day

Feel free to contact me I'm pretty aggressive with this the only thing I have not tries is PT.... I want some opinions on that?????

I think PT is critical. I don't think you can relax your way out of chronic prostatitis / chronic pelvic pain syndrome entirely. You have to break down the TP's that are causing pain. You may have some TP's in your perineum in the transverse superfical perineal muscles (about 1/2 inch in front of the anus). Check those out, and get a Theracane. It was perfect for that spot.

CPPS Dad,
Thanks for the tip (no pun inteded) -- but I have the same question as link. How, using a Theracane, do you get at the sensitive transverse superfical perineal muscles?

Many thanks for all your help. You guys have been a godsend.

Fasting

Post by Richie »

If fasting for a day helps, as it did me, check for food intolerance as a causative factor. Main ones are Wheat, Milk products.
Age: 44 | Symptoms: Pain in pelvis, Post urination drip. Helped by: Squat stretching, PT, Prelief before banned foods, Vit B, Cystoprotec AFTER foods, singulair after foods, Exercise. Makes worse: Stress, Alcohol (1-2 days after), coffee, Artificial sweeteners, chocolate, tonic Wheat/gluten. Drink a lot of water!!! 95% better to date!

First of all, most of us have far more trigger points than just the perineal trigger points. I had internal spots, abdominal spots, and perineal spots. It just turns out that the perineal spots were the worst for me with sexual arousal, ejaculation, and sitting.

To get to the transverse superficial perineal muscles

Image

...you need to press then round end of the Theracane about 1 inch above your anus, and then press in, and down towards the anus. You'll notice in the pictures found at the link above that the spot above the anus has about 4 different muscle groups all coming together there, making it a hotspot for pain. Those are the muscles that are involved heavily in the arousal and ejaculatory process, and so any pain in that area is likely to be a culprit. I press VERY hard by the time I am done. I start softly, and keep increasing the pressure once I find the right spot WITHOUT MOVING the Theracane any more. At my peak, it REALLY hurts (but in a good way).

About two inches in front of the anus, you'll find a hard bone. This obviously can't be pressed on very hard, but you can use it as a reference point. Often times I press on this bone, and then slide the Theracane down it towards the anus, but keeping the pressure on the tissue that I am pressing on as I move down.

After pressing down, I also angle the Theracane more directly towards the bone, placing the tip of the Theracane ever so slightly in the anal opening. I work my way around 360 degrees, and press and hold on all the spots around the anus.

You can also get some relief by using your fingers, but it is hard to hold the positions very long. Keep doing your squats, and hopefully you'll find some success.

Also, I frequently do all of this in the bath, as I find it softens up my tissue a little bit. But in reality, I can sometimes get "better grip" on the skin if I do it out of the bath. Once you try it, you'll see why "good grip" is important on the skin, as it's the only way to get at the muscles just where you want them.

I hope that gives you some tips!

CPPS dad,
Thanks for your help again.

I'm unclear, however, on your terms "above" one's anus, and then "press down towards the anus. Can you take a quick second to elucidate?

By "above," do you mean an inch away from the anus and toward the testicles? And, if so, how then do you go "down to toward the anus" while staying in one spot?

Also, you say you use the "round part" of the Theracane. Aren't there seven round parts to it?

Do you use the bulb at the end of the hooked part, or the one at the end of the straight part?

Sorry for the specificity of the questions. But many thanks again -ds

As the others said, PT is essential.

The effect of erection on reducing urinary frequency has been observed before, particularly in the following paper, whose abstract I have pasted:

Evidence for involvement of the subcoeruleus nucleus and nucleus raphe magnus in urine storage and penile erection in decerebrate rats.
K Sugaya et al.
The Journal of urology. 159 (6), 2172-6 (Jun 1998)
PURPOSE: Micturition and male sexual activity require the lower urinary tract to function. During the sexual act, micturition must be inhibited and urine stored in the bladder. We studied the role of the brainstem in relation to both micturition/urine storage and penile erection in rats. MATERIALS AND METHODS: Wire electrodes were placed on the dorsal nerve of the penis and microelectrodes for stimulation were introduced into the brainstem in decerebrate male rats. Electrical stimulation was used to locate optimally responding sites by monitoring the isovolumetric intravesical pressure and intracavernous pressure. RESULTS: Electrical stimulation of the dorsal nerve of the penis, the subcoeruleus nucleus in the rostral pons, and the nucleus raphe magnus in the caudal pons increased intracavernous pressure, but inhibited rhythmic bladder contractions. Electrical stimulation of Barrington's nucleus (the pontine micturition center in the rat) in the rostral pons induced bladder contraction. Stimulation of the pontine reticular formation did not increase intracavernous pressure. Acute transection of the thoracic spinal cord eliminated rhythmic bladder contractions, but gave rise to sporadic increments of intracavernous pressure. CONCLUSIONS: This electrophysiological study demonstrated that the subcoeruleus nucleus and nucleus raphe magnus are involved in both urine storage and penile erection, and that their physiological functions are reciprocally controlled; so that erection leads to inhibition of micturition.

Basically, for some people, input to the brain from other pelvic organs "crowds out" the messages from the urinary tract. Done under guidance of an expert, and applied in the right places, TENS might help you.

Side note, Link: do you notice symptoms changing right before or right after defecation?
Started: Spring 2003; high urinary frequency and pain associated with bladder filling; urinary hesitancy; pubic/prostate/perineal discomfort; Helped by: trigger point therapy, Afrin nasal spray, Cymbalta, hydrocodone (small doses), distraction. Makes worse: sex.

I've found that the Y-stretch did wonders for my perineum problems (especially the transverse superficial perineum muscles for me - as these are my biggest trouble areas).

To do a Y-stretch you lie on your back against a wall with your legs vertical up the wall. You then lower your legs to the side (making a Y shape). I hold the stretch for at least 5 minutes.

Any more stretching or Theracane techniques would be much appreciated....

I'm in Maryland and have been seeing Dr Toby Chai who is very good for IC and CP. After a year and a half with him he referred me to his colleague Richard Alexander who more specializes in CP.

Alexander is doing some trial testing

1) With a medication for seizures and gout ,I think, I can update the specific meds when I get my notes

2) Also, I think he is injecting lidocaine jelly near or in the prostate to numb and see if that is where the pain is coming from.. Will keep you updated

Any thoughts on this???????

I'm unclear, however, on your terms "above" one's anus, and then "press down towards the anus. Can you take a quick second to elucidate?
Sorry. "Above" isn't the best word. I mean in between the testicles and the anus, right down the middle.
By "above," do you mean an inch away from the anus and toward the testicles? And, if so, how then do you go "down to toward the anus" while staying in one spot?
This one is hard to describe. Above 1 inch away from the anus in the perineum is a hard bone. I place the Theracane on this bone, and then press down towards the anus. If you place the Theracane right next to the anus, and press down there, the Theracane has a tendency to "slip in" the anus, and this isn't what you want. You want to get a good push right on the muscles without slipping in. So I press on the boney area, then while pressing and keeping pressure on the skin there, I push down the bone towards the anus. I seem to get best results this way. Note that I DON'T press against the bone directly, as this would just hurt a lot. I kind of slide down the bone towards the anus.

Also, you say you use the "round part" of the Theracane. Aren't there seven round parts to it?
The curved (hooked) end. Sorry again for the bad terminology.
Do you use the bulb at the end of the hooked part, or the one at the end of the straight part?
The hooked part.
