Early Signs of Flare Ups
I have noticed that there are some early signs before a CP flare up becomes unbearable. My two biggest pre-warnings are fatigue and a slight discomfort (burning/tenderness) in the tip of the penis. Usually the fatigue sets in the day before a flare up, even though I had a week of 8+ hours of sleep, and only awakening once during the night to urinate. The burning/tenderness of the penis usually occurs the night before a flare up. This is a new symptom that started only a few days ago. This new burning sensation does not hurt when I urinate, but only is sensitive to touch. Has any one else experienced these early signs of flare ups?

My early sign of a flare up (which I rarely experience now, and which never rises to the level of my early experience with this disorder) includes feeling a burning pain right across my frontal pelvis, from side to side, especially when I squat or bend over.

Good News! The great ProstaQ is BACK at last! You must try it.

My early signs also include a sensation in the tip of the penis - urethral/frenulum area - but I wouldn't call it burning: more of a clogging, clamping pain. The most prominent signal, however, is a tightening, niggling sensation around my foreskin after I retract it. This sensation is also 'mirrored' deeper inside the pelvis, and very gradually it develops into a strong tension or tootchache like pain in my perineum and penis.

Unlike many people, I have never had flare ups which have just happened for no apparent reason. The only thing which has consistantly flared my symptoms has been exercise, and the degree of flare up is directly proportional to the volume and intensity of exercise performed.
My only warning sign is that I'm dying to do some exercise

Age:39. Age at onset:31. Symptoms prior to treatment: Golf ball in rectum, severe urinary frequency (2-3x/hr; 5-10x/night); weak stream; painful ejaculation; coccygeal pain; tip of penis pain; general pelvic pain on left; testicular pain; supra-pubic pain. Current | Symptoms: Urinary frequency 1x every 2-3 hrs and 1-2 x a night; mild pelvic pain on left hand side (all symptoms still improving!)
Helped by: Trigger point release; avoiding exercise; pelvic floor relaxation; Neurontin decreased bladder sensitivity somewhat. Worsened by: Exercise; frequent ejaculation; ibuprofen irritates bladder. Made no difference: Diet; biofeedback; quercetin; Steroid anti-inflammatories; Elavil.

My early warning sign is a low grade fever and a headache with sinus congestion. I usually wake up in the middle of the night with the chills and alternately feel hot and cold. I can then pretty much predict what my life is going to be like for the next 4 or 5 days. (lousy)
Because of this fever, I was convinced for many years that there must be a bacterial infection involved but I have never had a positive culture ( I've had chronic prostatitis / chronic pelvic pain syndrome for 30 years.) At one point I was on Cipro 500mg (2x daily) for over a year and still had flare ups.
My uro. (Dr. Nickel) has explained that fever can result from either inflammatory or auto-immune conditions.

Burning
My most persistent symptom at this point is also burning in the urethra and a sensitivity to touch in the tip of the penis. I'm basically pain free otherwise with treatment ongoing but these symptoms remain. Does anyone have a thought about what causes these symptoms?

Re: Burning
One of the emerging ideas is that it is a persistent nerve pain , or Sympathetically Maintained Pain (SMP).ddarcher wrote:My most persistent symptom at this point is also burning in the urethra and a sensitivity to touch in the tip of the penis. I have aborted 2 flares as they were beginning in the last two years, each time with one hydrocodone 500mg tablet per day for three days. This tends to support a neurological etiology.

My early signs (which is starting again today) are the dull sensation of clamping in the urethra and burning in the legs.
I've always suspected that my condition may be more or a narrowing of the urethra but as time goes on I accept it as a prostate problem.
I was doing really well for the past four or five days. I felt back to normal until early this morning when the burning came back. Hopefully it won't last.

I have a baseline of pain and voiding symptoms which are fairly constant. My flare ups are in the form of a flu-like feeling which can knock me out for months.
