Pelvic Floor Therapy

Post by reliefhelper »

Hi, is there anyone who has tried Pelvic Floor Therapy, or Trigger Point Therapy, who would share their experience? Thanks, Kathy
I am relatively new to using Pelvic Floor Therapy for Chronic Prostatitis, and CPP. Tomorrow, I have my first patient and am asking you to share your experiences using therapy for relief, positive and negative. I want to help.

PFT

Post by reliefhelper »

I have been working for a group of three Urologists for a year and a half, and have been trained through our equipment provider and going to SUNA conferences. I have only used PFT for Urinary Incontinence on both men and women. I have been researching on the net, the possible uses of PFT for Interstial Cystitis and Chronic Prostitis, and CPPS. So, my experience as you can see is very limited. Tomorrow, I have my first Chronic Pain pt, a male who has tried everything and is desperate to try me. Smile. Any advise? Thanks for your relpy, Kathy

I would suggest 1) buying the Headache in the Pelvis book and based on your location, 2) getting trained by David Wise and his team at Stanford.

I agree. You are only 1 1/2 hours from Dr. Wise, and he has trained many therapists in the area. I've even seen one, and he was very helpful.

If I had to offer any thoughts, they would be:
1) Be understanding. This is very uncomfortable for your patient. It might hurt a lot. And the patient is probably very nervous and/or scared.
2) Use a lot of feedback from the patient. TP's (as you probably know better than me) can occur in lots of places, and while you will be able to discern some of them, you will also need to rely on feedback from the patient.

But I agree with webslave that you will be more confident if you meet with Dr. Wise and probably watch some of his therapists do the therapy. You are in an ideal location to get training from the source.

Another piece of advice,

Please please please do not tell him you will have him sitting at a desk again, and weightlifting and running marathons by the end of the summer.
I've seen several PTs, all very helpful, who think they can get me back to sitting again. While my pain is lessened, I still can't sit.

They should be aware that they are in this for the long haul, and need to participate actively, by working on exercises and stretches and relaxation at home, for the long haul.

That's my two cents.

Also, be aware that many of us tend towards anxiety, and with chronic, very painful, mysterious pain that attacks the genitals, anxiety is very likely to dominate.

Also, thank you for taking an active interest in our community. We really appreciate your help.

This is not medical advice, and I am NOT a doctor of medicine or a related field.
* Age:33 Onset: February 2004.
* 99.9% IMPROVEMENT in 2.5 Years with the first year being the really hard part
* Current Symptoms: Mild irritation of perineal muscles on occasion. Relieved for days at a time by a specific stretch (see below).
* Initial Symptoms: Terrible penile, urethral, rectal, and perineal burning/aching with addition afferent sensations.
* Current Treatments: Deep stretching of the legs and pelvis. Most effective: Deep psoas and levitar ani stretch using the first phase of the "pigeon pose" from Yoga. When a deep pulling is felt in the middle of the pelvis next to the upper rectum, symptoms are completely alleviated for several days.
* Past Treatments Hyperprotection of the perineum for 1.7 years, Walking, Rectal biofeedback, Stanford/Wise-Anderson Protocol, Conditioned deep relaxation practice, Men's Multi-Vitamin and an Extra B-complex pill, all seemed to help.

Kathy, that's great that you are taking an interest in male pelvic pain. My PT said that men do not get much attention, mostly the woman for incontinence. My advise is to contact Tim Sawyer, he is Dr. Wise's best PT. He has an office in Los Gatos, close to you. His phone number is 408-237-7365. He can direct you to published literature describing the Stanford Method or he may invite you up to Los Gatos for a visit to discuss CPPS. I attended Dr. Wise's clinic and kept his card in my file. Good luck and thanks for having an interest in us aching guys.
