Newbie here looking for your help

Hello - I am posting here on behalf of my elder brother who I believe has been suffering from CPPS. My brother is in India and he is 40yrs old and has been suffering from chronic prostatitis / chronic pelvic pain syndrome since last 3 years. He has seen many urologists and taken countless medicines but nothing has helped him so far. My brother is in Bihar, India that lacks good medical facilities and doctors, I think he did not receive proper treatment so far. He once went to Bangalore and once to Delhi (Escort hospital) for diagnosis and both places he was diagnosed with Chronic Prostatitis. He was given some antibiotics and some anti-depressant drugs that he took but did not help. So far he had numerous Urine culture/Semen culture/Ultrasounds but nothing abnormal was detected. One of the urine test did report some E-Coli however a follow up test did not show the same. Again, due to lack of medical facilities and infrastructure, I am not sure how much to believe on those tests. I think his symptoms have worsened over the time and having side effects such as depression.

I would like to help my brother and would like your suggestion as to where to start the diagnosis & treatment. I live in Saint Louis, MO and even willing to bring him here for diagnosis and treatment but I am not sure where to start. I know there are members in this forum from India and I am hoping they can share their experience of treatment & suggest me doctors/PTs/hospitals in India familiar with chronic prostatitis / chronic pelvic pain syndrome treatment. If he were to come to USA ( he has visitor's visa) , how should I proceed for his diagnosis and treatment? I have read about Dr. Anderson's clinic and also about Dr. Shoskes. Should I start with one of them or should I take him to local urologist in Saint Louis. Looking for your suggestions and advise.

First, he should have a proper workup done by a urologist who is familiar with CPPS. This will help eliminate prostate cancer, abscess, and other conditions as the cause of his symptoms. I would imagine that a urologist would want to do an Expressed Prostatic Secretion, perhaps maybe a transrectal ultrasound and maybe even a CT scan or something else. If these tests come up negative and the urologist determines he has CPPS, then you can move forward with chronic prostatitis / chronic pelvic pain syndrome treatment. I don't know of any uros in STL who have good knowledge of CPPS. Dr. Doggweiler is in Tennessee I think, Dr. Shoskes is in Cleveland. Those would be good places to start. In addition to ruling out other pathologies, these doctors would be able to give your brother a prescription for any needed drugs (e.g., Tramadol, alpha blockers, etc.).

If chronic prostatitis / chronic pelvic pain syndrome is confirmed as the diagnosis, I would consider sending him to the Wise-Anderson Clinic. If you don't have enough money, there are other options. Look in the therapist/doctor/clinic thread on this board to find qualified physical therapists who will be able to help treat your brother's condition.

Crithinker - thank You for your reply. TRUS was performed and results are in the link. I don't think anything abnormal was detected. One of the doctor diagnosed him with BNO (Bladder Neck Obstruction) and gave some antibiotics. I am not sure if it has anything to do with his symptoms. I am hoping someone who has been treated in India will share their experience before i pursue his treatment in USA. While searching on internet i also read about homeopathy treatment by Dr. Rajesh Sah in Mumbai India as advertised here http://www.prostatitis.com/app/online.asp. I wonder if anyone has any experience with it and what was the outcome? I look forward for your suggestions.

Hmm, I do know there is someone on the boards who is in India. From his posts, it didn't seem like there was much going on in India. Search for his posts and you should find some info.

Bladder neck Obstruction is a very real and valid diagnosis if done correctly. I do not see why the same doctor would prescribe antibiotics.
Please keep in mind that health care in the US is VERY expensive and I have not found mainstream medicine particularly useful for this ailment.

I believe Singapore is a much more rational place to look for treatment for a person on that side of the world.
