I can't fully agree here. The muscular path isn't the solution for everyone. Pain management with a responsible, multi-disciplinary approach - including meds - has been helpful to me over the long term. Cure, no, but it maintains a quality of life after other choices have been investigated.

Age: 26 Onset Age: 17 Symptoms: Shooting, nerve-like pains throughout the penis, which abruptly hit and leave. Testicular pain, perineum pain, burning/irritative urination, extended pain after ejaculation. Occasionally, some allodynia or ache in the coccyx/sacrum/thigh/buttocks/legs. Diagnosis: Pelvic floor dysfunction, degenerated lumbar disk, and mildly herniated lumbar disk. Helped By: Physical therapy, pain management doctor, hot baths, therapy pool, stretching regimen, breathing exercises, relaxation, distraction. Worsened By: Arousal/ejaculation (worst), constipation, panicking/obsessing, other triggers depend upon current symptoms. Tests/Prior Treatments: Too many antibiotics to count, multiple urine tests (all normal), testicular ultrasound (normal), bladder and renal ultrasound (normal), lumbar and pelvic MRI with and w/o contrast (revealed disk problems), Elavil 25mg (caused retention), Flomax 0.4mg.

Tramadol, opioids, mast cell degranulation

I've seen several men who have failed to progress at all. Almost all of them were taking opioids on a daily basis.

The facts are that:
  1. This is a condition in which mast cells are degranulated by neuropeptide activity at the nerve endings in the lower genitourinary tract (see studies by Theoharides).
  2. Any medication that promotes degranulation will exacerbate the condition.
So opioids help on the one hand by suppressing pain, but hurt on the other hand by promulgating inflammation.

A 2010 paper shows that hyperalgesia can be induced by opioids. Another 2010 study posits that Tramadol triggers "an opioid receptor-dependent release of histamine from the mast cells". And there are numerous other suggestive studies.

All, just checking in. Wanted to post that I have had zero symptoms in months......this is a success story for those anxious newcomers......when I first visited this group I was severely anxious .....as you can see from my initial posts. Either way......I was able to cure mine through healthy eating, stretching (which I still do on a daily basis) and reduction of stress. I also consciously relax my pelvic floor during orgasm and moment to moment relaxation. Not sure if the fish oil has helped but I take it every day.....call me a fortunate one......either way I am ecstatic that this part of my life is far behind me. Age:31 | Onset Age: 31 | Symptoms: minor lower back pain, perineum pain, rectum, testicular....I have to shake and push a lot at the end of urination, but have had to for years. My initial onset was April 3rd 2011. I had major testicle and lower back pain. It has subsided to minor. My biggest problem is pain for days after sex. | Helped By: Eating healthy, hot baths, external PT, therapy, not thinking about it..Any other suggestions? I'm a bit hesitant to do internal PT.... | Worsened By: sex, alcohol, salt intake, stress/anxiety, sitting....biggest flares come on weekends after sex/alcohol and eating wrong....I am now cured as of May 20th or so from relaxation, healthy lifestyle, no stress/anxiety, eating properly, prosta Q, fish oil, external myofascial trigger point massage and TIME. Time heals everybody....Good luck and thank you all!
