After RMN, my neurologist realised I do not have anything on my spine. She listened to me carefully she even readed a bit of "HEadache in the Pelvis". She prescribed me Gabapentin 100mg and Escitalopram. I hope this will bring some relief to my neurological pain (pudendal etc..) while I keep working in my pelvic floor, stretching and pressing.
p.s: do you thing it is alright to also take MSM, Chondroitin , ... with this medication?

I have just sent the following e. mail to Dr. Wise explaining my case:
Dear Dr. Wise,
I will give you a little bit of my background. When I was younger, around 13 years old I was attending Scouts, that was on all Saturdays. Somehow events of frequency and urgency started to happen due to anxiety. I got scared with that feeling and all Saturdays the same anxiety/urgency felling would return. I was scared about this discomfort I started to feel more insecure because of this. I've then decided to leave Scouts, because was the only day I was having these feelings and I was not felling comfortable anymore. I had to go to pee in situations I felt where not the most appropriate, and the feeling of guilt and fear of someone to notice or to start making fun of me, or even to wet my pants was high. So I left Scouts. Some time passed without the feelings. However... I started to feel the same In school. during the week... and I started to feel afraid. "is this going to control my days again? I started to feel scared about it. and started to be very "aware of my pelvic area" and when to go to the bathroom. etc...
At this phase... my symptoms where very psychological... When I was in a relaxed environment like home... I would not feel anything... Or if a friend or someone would go to the bathroom... I would feel much more relaxed... Because I would think... "Nice I'm not the only one that needs to go, and I'm not going to be considered the one that has to go all the time". For me it was a victory to hold. Because I could feel more normal.
In my 20s I suppose, is when I started to have more situations of continuous discomfort. How can I make this affirmation? Because I started to feel sore even when I was at home. No relief even in a relax environment. So I passed from psychic to physical. This was the time I went to a urologist. They would prescribe me some antibiotic even without me having any bacteria on the tests. Things would get better... but they would also return. Bactrim actually usually makes me fell better... but it might be because of the anti-inflammatory effect.
I remember one episode where I went to do some bicycle riding (BMX) with a friend but he was a pro and it was very bad for my perineal area. I also by the time started to do more heavy weight lifting. Specially since the last 3 years and I believe there is a big correlation with this one and my problem.
I went to a psychologist 2 years ago or so, which relieved me a great amount of anxiety of going to the bathroom in public. However, by these time I could felt it was not just the psychic but also something physical.
Dr Wise, I do not know if I have any intolerance to food, any hernia or so. I do know I've been tensing this area for a while and that I've been doing some weight lifting that I am quite sure it does not help.
My main symptoms are now:
- irritation after a bowel movement
- flare up after abdominal exercise
- perineal discomfort that does not go away, especially just next to the anal sphincter side [irritation of the pudendal?]
- bladder discomfort that rises together with the quantity of urine inside
- discomfort when masturbating *which recreates a feeling of premature ejaculation
Are this symptoms typically linked to any muscle or trigger point?
Let me also tell you that sometimes I got better. Let me let you 2 times I remembered I had a significant relief.
1/ I did not masturbate for a long time, and I was probably not eating too much. These due to emotional reasons, because me and my ex. girlfriend broken up. Ironically, in terms of chronic prostatitis / chronic pelvic pain syndrome I was feeling 99%, I also cried, something I can't do easily. I think that helped me release a lot of tension by the time... I think I started to feel worst after sex, masturbation or exercise.
2/ I was doing internal massage on myself every day... and also related to the MSM tablets I was taking... *although I do not know if it was because of these... but I was feeling 100%. I was masturbating and I could feel... I'm a new man...
/////discomfort or not when masturbating and discomfort or not on the bladder when drinking a lot of tea for example, is the way I use to see what is my state////
I have a degree on Sociology, I have then decided to go to London and take a Master in Market Research. I worked there for a year and I am now working with my parents in sales selling ultrasounds equipments for Hospitals and clinics :)... I am happy with the job, ... However I can't live with this anymore... I want to feel secure about myself... feel happy... find a girls without having this garbage on my mind all the time... and move on...
So basically I am pretty close of leaving everything I have built in my job. and just aim to understand my body better for a while... because for now this is everything to me...
I do believe Dr. Wise clinic is the place where I can get the best advice, because you understand this condition better than anyone. I do have to admit that even if this is killing me I do still have to think about my savings... but I will have to decide very soon what will be my next step.
Dr. Wise does all this information ring any bell in terms of my diagnostic?

From what I have read so far... Dr.Wise protocol makes a lot of sense to me... but there are two problems... one is the money... the other is if I do not get any relief... My mind is going to be a mess...

In a summary way, I think I can explain my situation by "frequent and strong contraction of the bowel, which explain the post bowel movement irritation of the tissues involved and connected, post ejaculation irritation, and post heavy weight discomfort of the pelvic area"
Is there any lubricant that also have a relaxant effect or is more appropriate for irritated tissue, in this case, bowel tissue? That I may use with more success to release my contracted muscles?

Hi guys,
I need your help. On the left side of my internal pelvic floor I can feel some kind of tendon/vein/string. It looks detached from the muscles, it is like there was an "equilibrium circus string/chord" inside my pelvic area, that comes from I would say... close to the coccyx and would end close to the perineum... It looks very tensed... And not very anatomical... How can be something like that inside my pelvic floor? like a tendon without any muscle support...
I hope this make any sense....
I made a search now and might it be the tendinous arch of the pelvic area? Is it suppose to be tensed is that normal? Does that answer anything about my condition?
Also.. In one of my magnetic ressonance exams. There was found something on the letf sacral wing. I guess the bone is a bit oversized. Today I tried to reach the deep i could on internal massage. And on the right side I felt no pain. On the left side I felt pain... But this pain is diferent.. It looks more like bone pain then muscular... And it looks more dificult to release, if possible. I guess it might be related with that sacral left wing thing... Any ideas?

I think I have just found something.
My bladder was really full and that is when my symptoms flare the most.
First I lay on my bed and I pressed laterally my low abdominals. And I could fell pain... coming from the inside... like the prostate or so.
Then I decided to do some internal trigger point release with my bladder full. I tried to reach the deepest I could, and I could fell some muscles that I could relate directly to my pain. Then, and this is my discovery of today, I inserted the finger from behind, but this time not having the finger on a hook position, like I always do if I insert from behind. I was not going to massage around the coccyx this time. :alert:
So I insert the finger in a plain position, and I inserted it pointing to the center... and I probably founded my perineal area from the interior... It felt very hard... and when touched... I could correlate my pain very easily to that muscles.
I hope this is going to be the key to solve my problem... It makes sense if it is.
However it look hard to reach and release these perineal area appropriately. I will try to do my best though.

Yes, sounds right. But the hit-and-miss approach you're trying, without any training and without a purpose-built tool, is not the optimum approach. Good luck anyway.

Best way to hit the perineum internally is like this...
Facing your bathroom counter...(no socks on)
Lift your right leg up and with a gloved right middle finger lubricated insert by reaching around the right side, palm facing towards your butt cheeks and release the floor of your perineum carefully by pressing for 60 seconds. Make sure your left hand is supporting you by placing it on the counter...
Repeat with the left side...Left leg up, left hand and so forth...
Good luck!!!

Age, 44 onset age 37 Feb 2006 Freq. need to urinate. Sensation of having to urinate soon after going. Perineum discomfort/burning/tightness, pubic area discomfort @ times,poor urine stream, post urine dripping/spray. All symptoms have improved with my protocol. At the worst I give it a 1 to 2 on irritation and discomfort and frequency. Helps: Elavil 5mg for anxiety and mast cell protection, (will only take it as needed) self internal PT as needed, stretching, walking, stairmaster cardio workout and light weights, reducing stress, moment to moment relaxation, deep breathing relaxation and using a Theracane.
