You mention doing stretches. Are you doing any massages? For me, a lot of my penile pain is coming from my abdominal region.
This is NOT medical advice. I am NOT a doctor.

What kind of massages? I have no abdominal pain whatsoever it is isolated to my penis and perineum, mostly a discomfort of "pressure" in the perineum I would say. Makes it feel like I have to pee when I feel there is nothing in my bladder.
I do stretches not too many maybe just 2x 5 minutes a day.
I stretch my tighs, lowerback and perineum. Also when I do practice I stretch thoroughly for maybe 15-20 minutes mostly my lower back and legs.
Its so annoying since there really is mostly discomfort rather then pain and it fluctuates so much. Sometime sI can go 6 hours without peeing sometimes I can't go 30 minutes. I guess it might be a good sign, I do try to make the best of it and...well it will ease, as long as the penis discomfort eases I will be able to work on things again.
This flareup have now lasted almost as long as the last one and well..it will go away too and then I will start working on it with my PT again.
What also sucks is I dont have a tub in my new apartment yet so I can't have hot baths. That really didn't help me before but I still did it for blood flow reasons.
Sometimes when I go in a tub or have a shower if I have to pee a little, I feel the feeling get WAY worse when I am in there almost out of control.
Thats how it was last time too.

I've described the necessary self-massage multiple times in this post. I even listed all the different places you could work on. Please go back and read my posts again. Also, you seem to be missing the point of referred pain, something that is at the heart of what is going on. Whether you have pain in your abdominals, perienum, or pelvic floor is not conclusive - the referred pain may be COMING from there. All I know is that a good portion of my "front" pain (the penis and front of my testicles) comes from my abdominals, yet my abdominals are barely painful. Dr. Wise details this in his book as well. Treating the area that is in pain may not stop the SOURCE of the pain - and this is what you want to treat.
You seem to be repeating yourself a lot in your posts. I don't mind reading and responding because I want to help you, but you have to read the responses and be learning from them too, otherwise we aren't getting anywhere. I do want you to get better - take EVERYTHING people have suggested and try to build a solid routine. I don't know if 10 minutes of stretching per day will get you there, but at least it's a start. You probably need a little more, plus some trigger point therapy (from self massage), and eventually internal trigger point therapy too.

Thank you.
I do read the posts and I have reread them numerous times.
I have looked for a headache in the pelvis but havent found it in bookstores I have looked around quite a bit, I might order it on the internet.
I actually spoke with Dr Wise on the phone when I had this last time, after email contact he asked me to call him and he said he was sure stress is my problem and number one is to get rid of it.
He said muscle tension sure can cause all my symptoms.
Its jsut that this time it is WAY worse.
I am trying to understand this thing with trigger points, however I dont feel anything when I press on my abdominal area etc etc.
I get burning in my tighs and over my bladder on and off too lately but that I can take.
To describe the discomfort its mostly in the base of the penis, like a obstruction like feeling, that sometimes radiates down towards my butt.
Generally it makes it feel like I have to urinate, not in my bladder but in the penis itself which is strange and feel nasty.
All other pains I can live with but that one.
Anyway,
I am streching still, one time it got a little worse from it and I got a sharp pain inside me which scared me a bit.
It just feels impossible to stay possitive right now its just so blah.
Yesterday I tried, and I went for a supper I was invited for and it was hurting but I tried sucking it up.
It just needs to start easing..I was actually doing a bit better for a couple of days then it went back..but..well the flare up needs to settle down..then I will work with my PT.
Oh but on topic,
The triggerpoints you where talking too, and massages.
Just deep rubs of the thighs and abdominal area is good?
I dont mind trying that, I have only done the stretches.
Its just so hard when it feels like its actually getting worse.
Before I thought, when I had my good days..well better days, hmm its better then it was 2 weeks ago. Now its worse then it was 3 weeks ago so..I dont know.
PS
Can I do someting myself to stretch the levator ani?
I guess that is my problem since penile discomfort is number one for me.
Is there anything I can do right now, without a PT?
Thank you again.

Sorry for posting again but I have to ask something.
My wife gave me a through massage, and on my stomache deep in the "flesh" she found tiny little "bumps" that where the sizes of half a pea and rock solid and EXTREMELY painful.
She found maybe 5 or 6 of these.
One is right below and to the right of my belly button. Theyäre all in that area they just hurt when you press them.
I also had a few in my thighs, but I think those are from before.
We tried applying pressure to them and then letting go (Richie's tip) and we did that to the spots that "hurt". I might have had these spots even before I got this in fact I think so but it can't hurt to rub them can it?
Also I got and email from Dr.Wise saying I should try to take it easy and stretch and that people who's had this and been symptom free for a long time do tend to have their symptoms go away again, so I shouldnt worry.

My wife describes my internal trigger points as "little hard peas". If that's what your wife is finding, I think she is finding some surface trigger points. The fact that they are painful is a GOOD sign. My most painful abdominal trigger points are just off center (left and right) in the middle of my abdomen. Push HARD there with some direct pressure and see what you find.
As I've said before, in addition to stretching, you need to do myofascial release on your abdominals (and lower stomach area), perineum, hamstrings, inner thighs, front thighs, and buttocks. The levator ani is mostly internal I think, so I don't know how much you can do yourself unless you are flexible. But all of these muscles are helped by constant stretching - they are attached to other muscles - so you can help internal muscles by stretching all the external as well.
My best advice is to buy a Theracane. It allows you to do a strong massage quite easily, and hold it for a long time. You can lay on your back, watch some TV, and use the Theracane while you are doing it. I can get in 30-60 minutes of adbominal work that way. I also use it on my perineum, back, neck, and feet, to help relieve muscle tension. Relieving muscle tension sounds critical for you, because all of it combined (whether there are trigger points or not), are contributing to your problem. If you can relax the tension, you can get rid of the trigger points, and you can work on keeping them from coming back.
I'm glad your wife helped you out. In the end, you can do all of the massage work yourself, and not involve her more than necessary. And I strongly recommend that she becomes involved in any internal trigger point work you might need. Just offer her some of her own massages in return. And remember - this will NOT be like this forever. The intense therapeutic period might be short-lived, such that in a year your routine will be less.
The longer you stay in pain (it's been a while now), the less chance there is that this condition will get better on it's own and just "be gone". Don't keep waiting for this flare up to end. What's the point of that? Just get to it and start helping yourself. And get SERIOUS about it. I tried minimal stretching and therapy at the beginning, and it wasn't until I got serious and did if full throttle that I got better. Plus, when you do it half-way, and your condition gets worse or flares again, it's easy to say "the therapy isn't working" and just give up on it.

Thank you for your reply yet again.
Yes I dont know whether or not they actually are trigger points but we will keep pressing on them a few times a day and use massage therapy.
It can't hurt to try that.
I am trying to do streches for shorter intervalls several times aday like dr.wise recommended me to do.
Maybe 5-10 minutes 5 times a day or so is what I go with.
Plus I will try getting the massage in 2 times a day.
I think its good to be doing something, eventhough I am certain the flareup will ease up on its own, but if I can speed it up I will.
I am not quite sure as how to massage the perineum. It is quite sore and discomforty, it has never been painful though.
I have and I try to stay possitiv. Its mostly my urinary symptoms I need to rid myself of, the frequency isn't too bad I can take that but the penile discomfort is what I need to get rid off, well at least ease up. Sometimes its super bad and sometimes more manageble.
It will ease itll just take a little while but we will work on it...
Also today, after doing the stretches and getting the abdominal massage my symptoms actually worsened quite a bit, which is a little disheartening.
The spots that hurts the most are an inch or 2 below my bellybutton, one on each side maybe2 inches off center

Your sore spots are VERY near where mine are, if not in the same place. Mine are 2-3 inches below my belly button, and slightly off center in each direction as well as right in the middle.
Do NOT be disheartened that your condition worsened a little. This is 100% a good sign that you found one of your triggers. That's the hard part about treating yourself; it's too easy to get scared of doing things that are actually correct. Do not get scared off of the therapy. Just keep doing it and hang in there.

Thanks mate,
I will keep on doing this, the thing is I thikn I have had these spots since before I got this, maybe it just didn't manifest itself as penile discomfort etc until now.
I am SO sore today on one of the spots, but I have to keep doing it.
Also stretching I do now more throughly.
I am trying to relax as I thikn that is key.
I can't hurt it by pressing it too hard can I?

There is a solid wall of muscle in your abdomen, as my therapist explained to me. It would be pretty hard to injure yourself in the abdominals just by pushing on it. A good test would be to press in a non-sore spot the same depth, and see if that hurts. That would be your baseline.
Mike
This is the nature of this condition. Most of us tolerated the muscle tension and trigger points with little to no symptoms for many years before any tangible pain started. It just doesn't build up overnight.I will keep on doing this, the thing is I thikn I have had these spots since before I got this, maybe it just didn't manifest itself as penile discomfort etc until now.

Yes that is what I thought too, but working on them should ease this up soon enough, and taking it easy.
It just feels like there a "block" kinda in this one spot where I pee, in teh base of the penis and its nasty that is the one thing that NEEDS to go the others I can live with.
I will work on this..and take it easy.
So there is no way I can massage myself internally? Mostly I was thining the levator ani muscle

You could massage yourself internally using a tool such as the Crystal Wand. Others on the forum have reported doing internal work themselves (they have longer arms and are flexible). Before doing any of this though, it would be helpful to have a point of reference; I think this is very important. You need to know what it should feel like, and that means having someone with experience do it for you a few times. After that, your wife should participate, as she can make a big difference.

I don't think it's hard, but if you find any painful spots, you'll be more scared than useful. If you are even going to attempt to do this without at least ONE session by a professional, you should consider buying the latest version of AHIP (3rd edition), as it has a full set of diagrams. With that said, I did work on myself prior to any professional help, and I didn't damage anything. I've posted before, in fact, that no one here seems to recall doing any damage to themselves attempting self-massage (other than pulling back muscles). Still, it's worth it to fly somewhere ot have a session or two with an expert.
