Re: My story...What can I do?
I thought it was interesting that there was issues with bacteria. I'm not familiar with them but I'm wondering how they have played into this whole issue.
I continue to hope that you are finding ways out of this. There is much support here and Webslave is doing a great job manning this very important board.

Re: My story...What can I do?
Yes, I am also grateful for this board and webslave. Otherwise, I would think I was alone.
I think that bacteria could start a onset of this condition, or when you have chronic prostatitis / chronic pelvic pain syndrome you may be more prone to infection on days when your immune system is weak, but above all is the muscle issue. But again, I am no expert, just an expert sufferer. ;-)
Last night I went for a run of 10km. The whole run I tried to relax and drop my pelvis. Last quarter of my run I started to stretch my adductors in short breaks. At home while I was still warm I made many stretches and a bit yoga poses, then warm shower and I have absolutely no flare. Just don't do this HIIT Workouts with interval where you put your body to immense stress to burn more.
This is not for us with CPPS. Distance is ok, if you can stay relaxed. When I felt that my muscles were tightening (you get a feeling for this with our condition), I slowed down and concentrated on relaxing my pelvis.
On other days I love to have long walks.
What I know I will avoid for live direct abdominal workouts, they feel unnatural, too. Running and Walking feels natural.
What do you think about "side plank" position.
Is that okay for CPPS? And push ups. My brain says, they do not target the abdomen directly.
-----------
Alright, I think I swim one time a week for exercise I will be fine, even indoors. We do not have summer the whole year here, unfortunately.
My BMI is now something with 24. Way to go.

Symptoms: pain and discomfort perineum, a little bit while sitting in the right half of the scrotum. Sometimes dual stream while urinating. While ejaculating, not all shoots outside, some stays and flows outside with time (Without medication very painful).
Diagnosis: have seen several urologists in my life, have done any possible and pain creating diagnosis could not help me ever. Life was hopeful after googling the Stanford/Wise-Anderson Protocol in early 2008, then every ache started making sense./ Hemorrhoids: they stress my prostate when I have a Hemorrhoid flare!

Worsened By: Constipation, Fear or OCD "flare up", sitting and not doing any kind of "motion", too much tension in media or life, caffeine and acidy food

Alfuzosin vs Cialis
Hello there,
I have a return of my CPPS and neither Alfuzosin nor Cialis seem to make my pain in my tense muscles in the right buttocks, right adductors, right testicles, go away.
Alpha Blockers and Cialis seem both "heal" my post ejaculation/urinating pain. I cannot tell, which one is "better".
I also have ED, so would Cialis (daily use) be the better choice, since I just had to take one medication?
Or would you still prefer alpha blockers for CPPS?
Thanks for the help.

What do you think caused yours to come back?
I am also from Germany btw, I feel like no doctor here knows anything about this until I met some acupuncturists (doctor of traditional Chinese medicine). They certainly knew. But urologists, general health doctors, dermatologists etc aint know shit.

Re: My story...What can I do?
Why take both?
I rather have the medicine intake low.
Is one medication not enough?
I thought Cialis could address both CPPS BPH and ED...
So would you say, that one would benefit from taking both?
I would be so glad to take less medicine.

Re: My story...What can I do?
Sometimes medications are synergistic, so 2 together can be more helpful than one. But if you would rather take one, use the cialis first perhaps.

Re: My story...What can I do?
Hmm. Interesting thread. Yes, webslave and this forum are a great resource. I find it interesting that people get their symptoms from stress and other modals of over-exaggeration of physical activity. Please let me know how cialis affects your libido @User-CPPS. Stress isn’t a trigger for me nor are intense/ab workouts or the such. I actually love the stairmaster and ladder workouts in the gym and never get worse even a little from the use of the machines. I’ve never had my issue come and go. It’s always been persistent though the symptoms have only lessened since the onstart but the ED/libido issue has always stayed the same.. I’m thinking once I get over this condition it will leave for good. By God’s Grace.

Re: My story...What can I do?
It came back because of several reasons:
- I gained much weight! My BMI is about 30 now
- I changed from a standing job to a sitting job. I have a standing desk at work, but I may have not used it as much as I should have.
- I had much stress because of family issues, which was just before my onset.
- I had much stress because of frustration about my career and missed opportunities.
- I changed from a low distance job to a job where I have to commute. If I drive via car, I cope with jams, commuting with train takes too much time.
- My left leg is 1,5 cm shorter and thus my pelvis is misaligned, so my doctor suggested me to wear shoe inlays for compensation. My physical therapist said my legs are fine, inlays are a bad idea. So may be compensating on my left side was harmful.
- I try to loose weight in the gym and gain a better work-life balance, maybe find another job.

How to ruin your life making CPPS even worse with electro therapy
Hi there,
Since my CPPS relapse in 2018 I tried many things:
First, I had to struggle with myself to perform the treatment every night. I do not like to insert something in my anus but I got used to it. The device had two programs, one for urge and one for stress induced incontinence. When life was tense, I made a break from this treatment which made me go guilty to my next appointment. I swore to continue.
After I had a longer run and got more used to it the wire of the probe broke and thus I got a mild electric shock which scared me to hell. So I got a better probe from the company that visited my physical therapist.
Although that probe was much smaller it was a lot more uncomfortable because it used to “jump” out of my anus and not stick in place. After some treatments with the new probe I had a sudden sharp pain inside. I took out the probe immediately. That week I went to my proctologist who also was responsible for the electro therapy prescription. He saw some minor fissure inside and gave me suppositories. He also said that it is not clear whether the treatment was responsible for that or not. Next appointment he said everything looked fine.
Since then I have itching and discomfort feeling around my anus and that area of the butt cheeks around the anus. If I scratch it burns. I have difficulty to distinguish if the discomfort comes from inside the anus or if it is the skin outside because I cannot look at that part of my body that easily nor can I undress every moment to check where it is located. Some months ago I went to a dermatologist who could not help me. The proctologist does not see anything unusual either.
He wants me to continue physical therapy minus the electro work but I just feel uncomfortable and want to treat my discomfort first.
Also he prescribes me Diltiazem cream but with my skin condition I absolutely cannot tolerate that cream. I even told him once but he is so sure it would help but right now I just can't.
So I feel ashamed before my next appointment and still worry what is wrong with me.
Is it just in my head? Should I go to another dermatologist?
There must be something I could use to make that area feel ok again…
The upside of all this is that I have way better anal hygiene since these problems started. I use water after defecation and use toilet paper to dry myself. Also I trim all that hair once in a while. Before that I used too much toilet paper and used to rub very much until I thought I was clean. Wet toilet paper from the drug store would lead to burnings, so I used dry toilet paper only. The hair in that area was so long and thick that while rubbing the paper I always ripped hair. Showering to rarely in stressful times would lead to even more discomfort. As said, at least I feel that I take care of that body part now.
Why that dramatic title? I am responsible for trying this stupid electro Therapy. I still have muscle tension because of my CPPS but now I gained all those symptoms in the Anal area. So I ruined my health and have no clue what I can do about it.
Thank you for listening and helping.

Re: My story...What can I do?
Basically, you have an anal fissure, and nothing else can be addressed until that fissure is taken care of. Diltiazem hydrochloride 2% cream is used to promote the healing of the tear that has occurred in the skin of the anus and reduce the symptoms of pain caused by the tear. If you cannot tolerate Diltiazem, there is also nifedipine (also a calcium channel blocker that relaxes the anal sphincter and allows healing). Other alternatives are nitroglycerin 0.2% ointment or topical glyceryl trinitrate. So there are some alternatives to try. Pursue a cure for the fissure aggressively.
Good News! The great ProstaQ is BACK at last! You must try it.

Re: My story...What can I do?
Thank you.
But why did he not mention a fissure? He said that Diltiazem would help with the CPPS. I don’t have pain while defecating.
So you say the itching and the discomfort feeling (like sore muscles) com from a fissure? I also have stage 2 hemorrhoids and I loose water before defecting (which I hate).

Re: My story...What can I do?
He said you have a minor fissure that then disappeared. But he's given you cream to treat a fissure (that cream does not treat UCPPS). So you are getting mixed messages. And because he's the guy who gave you the script for electro therapy, he could be trying to cover his ass.
So I have suspicions that this goes beyond just a fissure, that you may have injured yourself internally, either through the insertion and extraction of the devices, or more seriously with the electric shock (burn?) you sustained internally.
I do not recommend using electrical devices internally, and it's not something I hear much about here, and it's not necessary.
I advise you to get checked out by another proctologist. The first one may be liable for any injury you have sustained. Get a second opinion on:

I advise you to get checked out by another proctologist. The first one may be liable for any injury you have sustained. Get a second opinion on:
- is there still a fissure internally? If so, could the device have caused it?
- is there any injury internally that could have resulted from a shock or burn?
Good News! The great ProstaQ is BACK at last! You must try it.

To be fair, he gave me that cream before I started the electro therapy.

Age: 41 Onset Age: 14, 22 got really bad
Helped By: Physical therapy, keeping myself warm, hatha yoga, stretching, walking, relaxation, distraction, Tadalafil 5 mg
Worsened By: Constipation, Fear or OCD "flare up", sitting and not doing any kind of "motion", too much tension in media or life, caffeine and acidy food
Prior Treatments: Antibiotics, Tamsulosin, Lyrica, Gabapentin, Tylenol
