It seems lately that my primary discomfort is just pain on the one side... particularly in my hip.

Does this indicate that is a problem of the muscle (rather than an intolerance to gluten and stuff).

Should I try a massage therapist, a chiropractor? The pain flares up when I sit. It feels like a burning sensation!

Any suggestions? Anyone?

Also after doing some thinking... I've been waking up in the middle of the night to urinate for years. Could it be I've had tight pelvic muscles for a long time and I finally had my first "flare up" (with actual pain, swollen prostate, etc)

Right now I'm waking up 3-4 times a night to urinate. Usually my average is 1-2x a night.

One sided pain is not unusual, and because pain can radiate all over the place your hip may not be the problem area, although it feels painful.

Please calm down. Your tone is highly anxious, which will only make things worse. Just accept it's going to take several months to fix this, perhaps even a year, and start making the changes that will get you there.

Keep away from chiropractors.

Hey Davyboy, hang in there. Listen to webslave, he knows what he's talking about.
And it sounds like you have a mild case that you are catching fairly early on. It's very important to take the steps you need to take to relax your pelvis and re-evaluate your life towards a calmer, less worrisome, less stressful path as soon as you can. Don't be like me! I ignored my symptoms for years thinking they would go away on their own, and I kept up my harried, worried lifestyle! I think I really twisted up the muscles around my prostate and abdomen bad and now they're taking forever to release! A couple of us are all tracking our progress together on this board, and we're all about in our 6-8 month stage of recovery. It looks like it's going to be a year or longer for complete recovery for me.
The first and best thing you can do, however, is get yourself a copy of A Headache in the Pelvis and read it cover to cover. The piece of mind it brings some men can reduce your symptoms right there! (It did for me).
P.S. Oh yes and my pain is almost exclusively on the right side of my pelvis, too. I don't think it's that uncommon to be one-sided like that.
Age: 35 Onset Age: 31
Symptoms: An almost constant feeling of the need to urinate all the time. Pain in the penis, at the tip, and "behind" the penis inside the pelvis. Also urethral pain after ejaculation. No rectal pain or symptoms at all.
Helped By: Hot baths, Dr. Wise's relaxation tapes. Internal trigger point work every two days with the Wise-Anderson Protocol wand.
Worsened By: Ejaculation (changes the pain to a burning sensation most times) Alcohol increases urinary frequency greatly. Falling "off the wagon" and not following the Wise-Anderson Protocol protocol.

I'm waiting for HITP to arrive in my local bookstore. I am looking forward to it.

I'm off to the Urologist this morning. Not that I believe I truly need it but rather my doctor referred me and I believe it will provide me with piece of mind that it is a muscle problem and not some other issue.

I have to apologize for coming on here and acting like a crazy person.

I've just spent the last few days laying on my back on my sofa trying to study and haven't left home much. So this has lead me to reading far too much information on the internet and spending too much time on this forum! I have to go out and live my life so my state of mind can improve.

Just got back from the Urologist...

He was great! He ordered an ultrasound and he told me he thinks its muscle dysfunction and is referring me to a physiotherapist.

FInally someone who listens ! What a waste of time and money on antibiotics, geez.

Sorry to bother you again... Just a question... Tonight I placed a heat pack on my perineum and I noticed it provided some relief (especially in my hip) but it caused a lot of pain in my left testicle. I then decided to place some ice on my testicle and the pain went away in literally 5 mins! Since you seem to know almost all cases on this forum, I figure I'd ask you directly if you've heard of this before.

Sorry again for my ocd I'm just struggling to identify what works for me and what doesn't. The one size fits all approach unfortunately doesn't work for chronic prostatitis / chronic pelvic pain syndrome and its slightly frustrating. Starting to realize patience is key...

Heat relaxes muscle, but ice can relieve nerve pain. Ice actually changes the number of some cytokines (chemicals that affect inflammation). This is why ice packs are so useful for back injuries.

A 2010 sports medicine study showed that in whole body cryotherapy (exposure to very cold air at -110C to -140C in special temperature-controlled cryochambers for 2 minutes), there is an increase in anti-inflammatory cytokine interleukin (IL)-10, and a decrease in proinflammatory cytokine IL-2 and chemokine IL-8. This offers hints as to why ice packs help.

Cryotherapy (cold therapy) is also used in patients who have had knee replacements because such patients show significantly lower pain scores than control patients.

Or is it the muscle contractions which are bothering the nerves?

(I know you're not a doctor, but you seem to be the most knowledgeable person I have access to!)

It's a :
  • muscular issue (hyperirritable, shortened muscles often containing trigger points)
  • nerve issue (overused, pathological pathways that crosstalk into adjacent structures)
  • immune system issue (pro-inflammatory cytokine profiles in affected areas)
  • endocrine issue (cortisol abnormalities; HPA axis)
  • CNS issue (central sensitization, windup)
  • psychological issue (hypervigilance, anxiety, sometimes OCD)
  • genetic issue (predisposing genetics)
If you are trying to understand it all before allowing yourself to start healing, that is not a good idea. It will all come to you as you read more and more here. At this stage, just accept the prescription and implement it. Men who have to understand or they won't stop worrying about it often fail to get better.

Would anyone be able to help me with a question regarding trigger points?

I have received my book AHIP and on page 275 (of the 6th edition), there is a trigger point shown in the bulbospongiosis and ischiovaernosis. I have been able to identify other trigger points in my body (legs, back, hips, etc) but with this particular one, when I push on it I do feel a sensation, but I'm just not sure if its a trigger point that needs to be worked out. Does anyone have experience with regard to this particular trigger point?
