Tascam, I'm not a MD and so I won't tell you not to get tests, especially if there are abnormal findings on TRUS. I would check it out further, based on the advice of a good uro, just in case there is cancer (unlikely, see what uros says).
I would rule out abscesses, based on yr comments.
Infection: no, not likely given symptoms. A couple of weeks of antibiotics like the ones you took should have brought symptoms to nearly nothing, but didn't, so no infection.
Most likely you are simply one of us, someone who "over-goosed" his pelvic nerves. Your sexual athletics overlaid on a susceptible pelvis is all it took.

I think webslave is right on. You have to think about the spasming of your pelvic muscles during orgasm. Combine that with any predisposition that you may have before to tensing those muscles. Those factors make you a good candidate to be one of us.webslave wrote:

I'm not an MD, but EPS seems to be the best method to test for infection (90-95% of cases of prostatitis are non-bacterial).
If you have the means, get the tests done that will make you feel comfortable that every avenue has been explored. I am in no way trying to give medical advice. I'm only coming from my experience.

I will quote the results from the TRUS report:
The CT scans only spoke of a mild prostate enlargement. I am going to a new uro on Monday and will have the results of a semen analysis and show him the TRUS images. I am still wondering if an MRI would make sense. I have read that with a PSA free score as high as mine (50%), it is generally thought to be a benign condition. I was told that PSA-F under 15% is more likely to be cancer. What do you think about that?Realtime Transabdominal imaging of the bladder demonstrates no intrinsic masses, stones or gross wall thickening. There is a minimum postvoid bladder residual.
Transrectal prostatic ultrasound demonstrates a diffusely heterogeneous but enlarged gland containing calcifications and measuring 2.8 x 4.7 x 3.7 cm. Correlating to a calculated glandular weight of 25 grams. There is a 7.4 mm hypoechoic solid nodule on the left, 9 mm hypoechoic right sided nodule and 8.6 x 8.4 mm hyperechoic right sided nodule all of which are nonspecific. The seminal ducts are symmetrical, each measuring 8.6 mm in thickness.
Conclusions:
Enlarged heterogeneous gland containing bilateral nodules. Two on the right and one on the left as described above. These are non-specific. The urinary bladder is unremarkable.

As Webslave has stated, he is not a MD, and neither are most of us posting. It would be ethically (and legally) dangerous for us to give you medical advice beyond a certain point, not to mention we don't really know enough to do so.
I would encourage you to keep your appointment with the second urologist and have him read your various test results. A second opinion from a fresh pair of eyes is never a bad thing. However, if the lesions detected in your TRUS were alarming, I have to assume that something would've been done for you by now. Prostate cancer is easily detected and highly treatable, so they'd have no reason to put it off if that were the case.
Having said all of that, as a layman, I agree with Webslave that you're most likely one of us. The locations and types of pain you have are classic. The fact that you have heightened symptoms post-ejaculation and under stress is indicative. Relief gained from hot baths/relaxation/Xanax is REALLY indicative. Don't panic, though. chronic prostatitis / chronic pelvic pain syndrome is not a death sentence, and you can get better with the right approach. It's gonna demand effort and lifestyle changes on your part, but that's a small sacrifice for most sufferers.

The last Uro wanted to do a biopsy on those lesions, but I insisted on a PSA-F test which is supposed to differentiate between benign lesions (as in BPH) and cancerous lesions. The PSA F score above 25% is supposed to indicate a benign condition. My score is 50% and that's why I hesitated to do a biopsy, especially with a PSA of 1.3. I am anxious to see if the semen culture shows any bacteria.

Tascam, did you try any stretching? I personally have found that stretch you do when you lay on your back and bring you knee up to stretch what I believe is the priformis stretch to be useful, also the one where you put your feet against a wall and slowly move them apart, holding for the stretch to be helpful. another thing I read on this forum was to do gentle squats no resistance, just up and down. When I was battling frequency, generalized pain in pelvis, these were very helpful to me. Another thing I realized to as that as I was moving forward and dealing with the problem, I have forgotten what exactly it feels like to be "normal." concentrating on this, stressing out about it and worrying really will make things worse. Your body goes into a hyper vigilante state and keeps you on edge (I was in this state for a while and it took me deeper into the pain), this will prevent any recovery. I know its hard to do, as was for me, but just relaxing, especially while you are awaiting the results will help you deal with the stress. I was in the same boat waiting for my results. It ate me alive, as this is before I knew about medications to help with it, etc, etc. You'll be ok man, things will get better. Whatever the test says, remember, you are your backup plan. No matter what the result, there will be ways to deal with it and you will get better. If its bacteria, youll take a assload of antibiotics, if its not, well youll probably stretch and work out physical therapy, or take muscle relaxers and deal with relaxation. But I know even in my worst flairs, what gets me through them, is knowing that IT WILL GO AWAY. granted, it will come back, but knowing this makes all the difference and keeps me out of the abyss.
You had mentioned that Xanax helps you in your signature, now I'm no doctor, but I have both Xanax and valium on hand. Xanax is ten times stronger than valium, however, its half life in the body is not as long lasting as valium. You may want to talk your doctor and see if you can get something that will be effective for longer should you need to go the muscle relaxer route. Also, youll be more prone to rebound anxiety from the Xanax as its half life is shorter. Just a suggestion.

Tascam, I suggest you do the biopsies for peace of mind. Once that issue is settled, you can move forward with a clear head. The threat of cancer must not be taken lightly, especially at your relatively young age.

I will see what the new Uro says on Monday. Also, I have an appointment at University Of Miami on December 15 so hopefully, within few weeks, I will know for sure what I am dealing with. It's immaterial what we think about prostate cancer. Almost none of us has had it and we are not urologists. While your tests indicate a low risk of cancer, you need to heed the advice of your specialist doctors. Having incurred over $100,000 in negative tests I can tell you that Webslave is probably 99% correct. You simply overgoosed your pelvic muscles and nerves and they have not recovered. I have been dealing with it for over 2 years and am not any closer to resolving it. I do believe the more you mess with it with injections, etc., the worse you may make it. However, I understand how counter-intuitive doing nothing may be. There probably should be another warning on Viagra . . .
