To address the original question, it sounds like the injections were simply anesthetic with a long-acting steroid. Neurolysis would not have been performed on the pudendal nerve, as it is not solely a sensory nerve, but one which also controls bodily functions.
Regarding nerve medication, I can only reply that I have been on Lyrica 300mg/day for years without any such issues. There was drowsiness at first, but that has faded. Now, it may very well have caused some reaction for you. Medications do strange things like that. An Advil that relieves one person's headache can kill the next.
I can't address your statement about PNE, as I do not personally believe the condition exists outside of some direct trauma to the region. The peer-reviewed medical literature just isn't there even after years of surgical intervention, nor are there adequate follow-up statistics on resolution of symptoms, and either factor would be enough to put me off. What's more, there are plenty of folks who report unchanged or worsened pain following decompression of the "entrapped" nerve. I think the widespread sensationalization of PNE on the internet is an invention of the few surgical groups actually willing to perform the surgery. So, I suppose I'd be biased in that regard, yes.
Sorry that you feel you received no benefit from our forums, but thank you for returning to answer the poster's questions. I hope that you are able to find a solution to your problems at some point.

I am not a physician. This is not medical advice. Consult your doctor!
Age: 26 Onset Age: 17 Symptoms: Shooting, nerve-like pains throughout the penis, which abruptly hit and leave. Testicular pain, perineum pain, burning/irritative urination, extended pain after ejaculation. Occasionally, some allodynia or ache in the coccyx/sacrum/thigh/buttocks/legs. Diagnosis: Pelvic floor dysfunction, degenerated lumbar disk, and mildly herniated lumbar disk. Helped By: Physical therapy, pain management doctor, hot baths, therapy pool, stretching regimen, breathing exercises, relaxation, distraction. Worsened By: Arousal/ejaculation (worst), constipation, panicking/obsessing, other triggers depend upon current symptoms. Tests/Prior Treatments: Too many antibiotics to count, multiple urine tests (all normal), testicular ultrasound (normal), bladder and renal ultrasound (normal), lumbar and pelvic MRI with and w/o contrast (revealed disk problems), Elavil 25mg (caused retention), Flomax 0.4mg.

Can't agree or disagree as far as others are concerned about PNE but I do believe I have it, and YES, I have had direct trauma along with other factors that would certainly meet the predisposition to have nerve entrapment in the sit bone area. 1) 130mph moto fall that left me with 20 broken bones including severe compression fractures of T11 and T12 which left me with 6 fused vertebra in Herritgton Rods 2) Since then, my hips are permanently tilted/twisted/rotated 3) 10,000+ miles on a bicycle 4) 150,000+ miles on motorcycles. 5) And not sure if it's related to impingement or not but I also have tendon issues in my groin/pelvis area.
Yes, if I had to bet on it, I would say that I have PNE under (someplace near) my left sit bone. I can actually feel the nerve irritation in that area and if not appropriately addressed the pain becomes completely debilitating.
By the way, the other thing that never worked for me was heat, hot baths or anything of the sort, but exactly the opposite. Ice-packs are my saving grace and what keep me from having to medicate constantly.
Oh, and before the coma (and almost dying) I actually had an appointment with Dr. Hibner in Arizona. The actual appointment would have been about a few weeks after I got out of the hospital. Was going to see if I was a candidate for his decompression surgery. That appointment was cancelled and no other was rescheduled. Not sure I can put myself through that at this point.

Age:43 Onset Age:33 Bacterial Prostatitis lasted about 1yr, remission, then back in and out of nonbacterial ?? (whatever) for years. Never nearly as bad and constant as now (last few months) Symptoms:Burning perineum/urethra, Urgency, Weak urine stream & spray, Post urine dripping Helped By:Lying down (preferably on back) Worsened By:Sitting. I used to only hurt when sitting for prolonged periods in certain chairs and motorcycle seat. Ergonomic bike seats used to be A-OK. Standing and or hiking always made me better. In the past 3 months I have rapidly declined. Everything hurts, sitting, weights, work, hiking and now even sometimes standing provides no relief. Almost constant pain of some degree unless I'm lying down.

You are ready to bag this forum, but the only things you have tried are things we specifically advise against (injections) or on which we have no opinion (Neurostim). Amazing.harpo-rider wrote:Sorry I can't be of more help but I'm not aware (no longer remember) what Dr. Weiss's concoction was. I do know that there were supposed to be several ingredients that were supposed to help short term pain, swelling and gradually reduce long term pain. (steroids, cortisone, ???) I stopped seeing Dr. Weiss quite some time ago and have been through Stanford Urology for Neurostim Implant, (implanted and removed-made me worse) Stanford Pain Management Clinic for numerous different injections, John Muir Pain Management Consultants also for several different series of injections, and so far absolutely nothing has helped.
Swine flu.Next/last/worst, July 20, 2009 I had my friend rush me to the ER after being sick for about 6 days. (and all ready on antibiotics/puffers/other meds for 3 days) Long story short, both lungs were in the process of shutting down. I was put into an induced coma (22 days) that I was not expected to awake from with about 15% lung capacity. I survived, Yea!!! 28 days after arriving at the ER I left the hospital.
You have your own theories for everything. My experience is that people who insist on "re-thinking" their conditions in unique ways, following their own layman's intuition, have LESS chance of full recovery. Usually it leads them to believing in the infection fantasy, but sometimes we end up with a case like this.My GUESS, the nerve meds had/have a lot to do with all the connective tissue damage and what was diagnosed as Multilobar Pneumonia w/ARDS and Acute Respiratory Failure.
But you say in the same breath that you never spend a moment without reminding yourself to relax your pelvic muscles, so clearly you are very confused.I got no help what-so-ever from this board ...
Well there we have it. A flooring installer who knows it all. You haven't given any of the things we recommend here a try, but you've already decided we're wrong. You went to Weiss rather than Wise, against our recommendations. You believe in the PNE myth. You elected to have injections, which we counsel against. But we never helped you. Great stuff! At least in my case, is that if you truly have PNE (which I’m 99% certain I have)
Good News! The great ProstaQ is BACK at last! You must try it.
