Why apologize for asking the questions? Questions are a good thing.
Here is what I can tell you I experience when hitting the trigger points. First I hold them at least 3min, but upwards to 5 min. Usually I must hold 1 & 11 o'clock up to 5min. I do believe there are times I feel something release around around 3 & 5 or 7 & 9 o'clock, but usually in those areas I just feel things less painful the longer I hold. However - 1 & 11 o'clock are a different story. In those areas I can refer pain to some areas that cause the flares including the feeling of urgency. But I don't feel it actually release. When I stop, the pain and urgency may subside within minutes of finishing up. Sometimes I will release much more urine afterwards. The problem is, sometimes I feel better the next day, or it causes my penis and urethra to flare up the next day. So either that is a hot zone, or I am doing something wrong at 1 & 11 o'clock. Yes I am relaxing, and doing the belly breathing, but will admit sometimes my mind gets distracted and I start losing my concentration. I set up a bedroom in my basement, with no tv, and I usually put on something calming and tranquil like music with rain or water running down a stream to help calm down.
Regarding my diet, I swear I can no longer tolerate eating most fruit anymore, and I love fruit as well as most veggies, but veggies tend to get boring. Sometimes I would like to snack on some fruit. I can tolerate blueberries. But everything else will bother me in some way or another. I'm not sure if it's the acid, but I am starting to wonder if fructose is a problem.

My theory, and I believe I am the only person on Earth to have formulated this theory, is that sugar consumption (and especially fructose) increases blood levels of uric acid(1,2) and causes a spurt of uric acid to be excreted via the kidneys/bladder/prostatic urethra, so causing discomfort to the inflamed tissues there.ChgoGuy wrote: Sun Oct 14, 2018 2:49 pm I'm not sure if it's the acid, but I am starting to wonder if fructose is a problem.

Oh I don't doubt your theory at all Webslave! So then what are we missing? Are the doctors this ignorant that when you tell them your urine may be burning your urethra, they don't bother to check for uric acid? I've probably had over 15 urinalysis done in 3 years. It used to be just orange juice that used to only slightly irritate things for maybe the 1st few pisses afterward. The uric acid theory too has crossed my mind, but why is it so hard for them to want to check that?
But my sugar consumption is only a fraction of what it used to be. I really do only drink water 99% of the time, and I thought that would help, especially by diluting the urine.

I came up with the theory many years ago, while I still had CPPS, when I noticed that my bladder burned after eating lots of sugar. There had to be a reason, so I went looking for it. It could be a subtle shift in urine pH, not worth the doc's while to check.

Well I keep telling myself lately that sugar is the devil. I think it sucks that I have to fear eating fruit. I find that if I cheat and lets say have one cookie, that doesn't appear to bother me. But if I ate an apple, it not only bothers my bladder but has a negative effect on my bowel movements. Like giving me very loose stools which also tend to irritate down there more than harder stools. Very IBS like in nature.

Great thing about it now you know which foods you can stay away from now thru trial and error. High acidic foods bothered me! Especially anything citric. Keep the food journal.

I have an Amitriptyline question for anybody that wants to answer it. In the last week, I started taking 10mg of it again at bedtime. I've never taken more than 20mg before. I feel like it does help for at least a couple of weeks. Maybe I should have upped the dosage to 30mg at that point, IDK. But one thing is clear, I don't want to take anything. That's besides the point.
So here is my question, after about 16 to 20hrs, things start to bother me again. So would it make sense, instead of taking 20mg at bedtime, to take 10mg every 12hrs (or split the dosage of whatever mg in half and take 2x a day instead of all at once)? I'm just wondering if the effect is wearing off to early in my day, which is at a time when I may need it most. Of course drowsiness worries me, but it's such a small dosage anyway.
Opinions welcomed...

I take amitriptyline 50mg for a year now (last 2 months 75mg) so I know some things about it. The effect is indeed wearing of during the day. I take my 75mg at bedtime, but my doctor said that I could also split them for each 12 hours. But, be very strict about it with the intake. Always at 8/9/10/11 o'clock, not 8 o'clock in the morning and 11 at night.
When I was taking 50 mg, I could easily ride a car. For the first 2 weeks it is a bit of a strange drowsiness feeling, but this goes away quickly.
It is nasty stuff so stay away from it if you could, but it is not the nastiest medication out there ;)

Thanks MrDavid! Generally my days begin at 4am. So if I take mine in the evening about 8pm, is seems to be useless between noon the next day and 3pm, which is before my ride home. Thankfully my ride home is under 30 min. But then my evenings suck, and when I take it again at 8pm it doesn't kick in until I really doze off.

Well for me it is not entirely useless at night. It helps me to get a deeper, better and longer sleep :)

On another note. I hurt my lower back again at work recently. I went to PT for that today where my job sent me, and I think that really aggravated things. I tell them there are certain exercises I cannot do that will cause a flareup, especially in the core area. He had me ride a bike for 8 min, and some time after I was done and left, it felt like somebody was punching me in my perineum. WTF!!
