Nick, I use the wand to go in and stretch the sides of the prostate, then go above the prostate. Basically just feeling for any tightness. I focus on holding stretches versus working trigger points (I don't think I have any trigger points). I started with fairly gentle pressure, but have worked up to a firm pressure. On the wand gauge its around 50 or so. I don't recommend this (you will figure out your own pressure needs). Just what I do. I hold stretches for about a minute at a time maybe longer. I also "sweep" the prostate. Tim Sawyer suggests that some people need to do a lateral to medial sweep of the prostate to hit the insertions from another angle. Light pressure on the sweeps until you get good control of the wand.
Now, without the wand, I would suggest gloving both your hands, getting some gel lubricant, put your right foot up on the bathroom counter, reach around with your right hand, insert middle finger (lubed). At this point, just notice what you notice. See if you can gently feel your prostate (what's that feel like etc.) If you can feel your prostate lightly stroke it lateral to medial. Explore a little bit. Err on the side of less versus more pressure at first. Just get a good sense of what it all feels like.
But, what has worked for me is hooking my finger just inside the rectum and pulling down and out...and just holding this. This indirectly stretches those insertions behind the prostate. Repeat for the left side. I've done this every day for at least a year and a half.
Now, if you are adventurous, I like to get up on the toilet seat (squat--feet on the actual toilet seat---there's an image on here somewhere, but, you know, as if you were going to have a bowel movement Asian style). This takes a little balance and you may want to do it against a wall if you are heavier. Anyway, when squatting on the toilet seat, the pelvis can really relax. In the squat, I just reach around and grab my right cheek with my right hand, and pull down and out (I don't go internal...just grab the cheek). Hold. Repeat. Do the other side. I usual do this post bowel movement. Since I'm already in there. Helps with any post BM issues. I actually stopped all sitting on the toilet. I only squat for a BM. Webslave had some good comments on this. Never occurred to me before dealing with this. Squat on......

WHOA. You use pressure on the wand gauge that measures FIFTY!?! Holy crap that's an enormous amount of pressure. The guide that Dr. Wise includes with the wand says that under no circumstances FOR ANY REASON should the gauge reading ever go above 37 MAX.

Whoa yoink, its all good. Its not an enormous amount of pressure. And what Dr. Wise says and what Tim Sawyer recommends do differ. And, my guess is, Wise hasn't really used the wand all that much. Just my guess.
The great thing about this forum is you get an anecdotal look into what people do and what is and isn't working. Its all individual. Wise has to have disclaimers or he's gonna have all kinds of lawsuits. I did disclaim it by saying I don't recommend this amount of pressure. Its my thing. You gotta ease into all of this. I only mentioned the pressure to let people know, if they have concerns about hurting themselves, that I do use significant pressure and have had no issues and to the contrary am pretty much recovered.
I will repeat...be gentle at all times on yourself. Don't go in there like Van Damme. As you get more comfortable, explore a bit with pressure, depth, etc.
And the final warning, do not use this kind of pressure directly on the prostate. Easy does it going between 11 and 1 on the pelvic clock. You do run the risk of injuring the prostate and the urethra if you grind at 12 o clock. This is laid out in hitp. And is fairly common sense.
Interestingly, if you search for some of neue01's posts ( I think that's his handle), this kid was going way up into the great unknown. He was talking about 13-14 inches. So. I mean. I don't know. He seemed to think his issue lay up there. Art more than science.

Thanks WTH for the description of what you do. I have done a massage session, I will keep on going and see what happens. It is not easy with just fingers though. I am still not exactly sure I have found my prostate though, I know roughly where it should be. I basically go in from the front and insert my index finger past the sphincter and then bend it, I feel the walls of the rectum, right at my finger tip I can feel a slight soft ridge, like a fold of skin/muscle, I can't apply and pressure in this area as it is at the limit of my reach. I think just before the ridge must be my prostate, I can feel something when I press on it, it is not painful though. The funny thing is no doctor or urologist has been interested in my prostate at all, never once did I get a DRE, which I one thing I was expecting but not looking forward to. My urologist jumped straight into a cystoscopy, maybe that pays better?
Age 40. Symptoms started Feb 2011 but for a number of years before sometimes had burning pain after ejaculation for 30mins or so. Initially testicle discomfort, then bladder pain/urgency followed by penile discomfort and burning after urination. Used to have periods of being symptom free but now have continual penis discomfort, mainly at tip. During a flare pain is more of a burning type, especially after urination. 3 negative urine tests, cystoscopy, ultrasound of kidneys/bladder all negative. Urologist never gave me anitbiotics or suspected prostate infection but couldn't find out what was wrong. Helped by rest, heat (sauna/hot bath). Just started on amitriptyline, no benefit yet.

The physio Fetske Hogen Esch seems to have a lot of pelvic floor experience. I'm thinking of attending, at least for an evaluation. My symptoms are constant feeling of needing to urinate and some mild redness around urethral opening. Also had some testicle pain about 4 weeks ago but now gone. Had the usual bout of tests urological tests but all clear for infection. My DRE was painless. Been like this for about 12 weeks now.
Any advice much appreciated!
