Another thought
I am open minded and willing to consider that my problems are trigger point related. However, I do believe the initial phase was acute and if I had been properly treated it would have resolved. Irrelevant now I know, but I have been doing a little research on a drug I have seen many of us have used, hydrocodone with acetaminophen...Here is a listed side effect of the drug Genitourinary side effects including ureteral spasm, spasm of vesicle sphincters, and urinary retention have been reported. My primary symptom has been scrotal and testicular pain after urination, unfortunately for the last week it has been constant pain....I can't find a definition on spasm of vesicle sphincters, can anyone shed some light on that? Before the constant pain started again this week it literally felt like urine was somehow leaking to my prostate or wherever and initiating the pain.
Secondly, my doctor has suggested a colonoscopy, I have procrastinated for a while because I saw no point and didn't want to pay for it, as I have a high deductible. My wife has been pretty persistent that she would like me to get it done so I broke down and scheduled appointment for Monday. Has anyone heard of anyone who actually had something found that helped resolve this during a colonoscopy?

Age:38 | Onset Age:37 | Symptoms: Pain primarily in right testicle, greatly worsened after urination although recently whole scrotum feels inflamed again..no urinary issues..this is my third and longest bout in last 8 years...Never have burning in urethra after urination but scrotum and rectum will feel inflamed afterwards.
Helped by: Warmth, walking, valium some, hydrocodone sometimes helps
Worsened by: Sitting, pain day after sex, bowel movements, sugary foods, seemingly ibuprofen and advil?...supplements like vitamin d, quercetin, saw palmetto cause worse pain, certain antibiotics make pain much worse as well.

Spasm of the ureters (channels leading from kidneys to bladder) can result in difficulty urinating (rare side effect).
The "urinary vesicle" is also known as the bladder.

Okay I put in a signature, so basically the side effects mentioned would have nothing to do with urinary reflux into prostate, correct?

Your chronic prostatitis / chronic pelvic pain syndrome is centred on the scrotum, not that unusual, and it's often called orchitis (search the forum for that term), while some call it epididymitis (another term to use for searching).
Urinary reflux into prostate would not account for those symptoms, IMO.

Well as I expected the colonoscopy was 1k down the drain....so the scrotal and testicle pain is something that typically is helped by the trigger point etc? I've been starting to wonder if it is ic because of the pain involved in ingesting certain vitamins, pro-biotics and foods which I understand is common with ic....however I have no frequency or urgency issues and the pain is worse after I urinate not better which I understand with typical IC patients its the opposite. I recently discovered there are some pelvic floor therapists in the area by reading an article about their therapy with women..I feel like if I call them and say my balls and scrotum hurt can you help, that they will think I'm some kind of pervert.

It would have been better to spend the money on a cystoscopy rather than a colonoscopy. They would have visualised your urethra, prostatic urethra, bladder neck and bladder. Have you seen a urologist yet?

I saw a urologist who did a DRE and examined the prostatic secretions under a microscope and said there was inflammation and said I had prostatitis and prescribed more antibiotics coupled with Flomax. I only took the Flomax briefly because it didn't make any sense to me because I didn't have any urinary symptoms...I figured a cystoscopy would be the next step. Would you do that before calling the pelvic floor therapists? Would you agree with my assessment on IC?

IC usually involves painful urination. I think you would benefit from a cystoscopy to rule it out, if your insurance will cover it.

I have a high deductible so I would be paying but am willing to fork down money for anything that will help. I'm really leaning to the pt though because what seemed to help some was the valium and I am prone to tightness in general.

Try PT then, and if it either makes it better or worse, you know the pelvic muscles are involved. (Note that PT can cause a flare in the condition if trigger points are palpated).

I'll give it a shot, I live in a relatively small population center....I did see that we have pelvic floor therapists, ironically in my neighborhood, but on their website of conditions treated prostatitis is not mentioned, so they may turn me away. It also looks like they work mostly with biofeedback....thanks for listening.
