I'll jump in here and mention that loosening up and healing the internal muscles takes a lot of time, effort and dedication. Flare ups take days or weeks to die down especially in the beginning while working on internal muscle groups. They can be the worst too because it makes you feel worse and has you questioning yourself and the condition a lot. At least it did for me. It has taken me 8 months or so of almost daily massage to break loosen up the area. I began to see progress with in the first month but it took a long time to really dismantle the many trigger points and areas of tightness and congestion. I had had this condition for many years prior to doing internal work so that could have been a large part of my situation as well. It's hard to say but I do know you have to work hard and give in some time.

Webslave made the following comment to someone who experienced urethral burning after urination and feeling of a UTI after ejaculation. What I would like to know is what we should do when we experience this? Should we search for trigger points to release or would that be considered "neural work" that would cause more symptoms? I am experiencing something very similar and would like to know how to proceed. I also have a feeling of bladder related pressure in my left lower abdomen. (It was hacksquat who posted the problem that webslave responded to.)

Nerves. The ejaculation goosed the nerves ennervating the pelvis, so any neural "work" in the area causes symptoms now because of a process called allodynia, in which normal sensations are perceived as painful.

When I referred to neural work, I meant that the nerves are heavily used (eg during orgasm/ejaculation). PT is quite different. You are not firing up the pelvic nerves when a trigger point is being massaged.

You ask how to proceed from here. The whole forum is about that!

Hi,

Citing HITP,the following can be responsible for frequncy, urgency, and/or urethral pain:

Anterior Levator Ani, Superior Portion (aka Puborectalis) -- one of the most important trigger point sites for men
Anterior Levator Ani, Middle Portion (aka Levator Prostatae)
Obturator Internus

Flares after orgasm are quite common, partly because of muscle contraction, partly because of neuropathic excitement (I believe). Hope this helps answer your question!

The hyper-irritable spot that felt like salt in the wound went away with self-treatment with a tool and all of my scrotum pain disappeared right along with it. Thanks all for your input. This condition is very weird and it's a shame more doctors aren't aware of what can be done to treat it. For every one of us on this forum there are probably 9 others out there with this pain who don't have a clue what to do.

Jay or anyone else - Would you tell me how to best reach the anterior levator ani, Superior Portion?
About how many inches in is it rectally? I understand that anterior mean it will be towards the front, or belly, but as far as as the clock comparison goes where should I search? This thing is driving me nuts.

Yes, I do. I have bought every edition and now have the 5th Edition. It's my pelvic pain bible. I do have some trouble figuring out the diagrams though , because of course it cannot be 3D and they have to "flatten" everything out. So that's why I asked how many inches in the anterior levator ani is.

My latest flare has lasted a week now. Sometimes the burning turns to itching. I was doing great until I overdid the sex.

I welcome all help and suggestions and clarifications of how to locate this and treat it.
